摘　　要：目的比较神经内镜下经鼻蝶切除术和显微镜切除术对垂体瘤的临床疗效。方法选取2014年6月至2018年6月在铁岭市中心医院神经外科治疗的50例垂体瘤患者随机分为两组,对照组30例采用显微镜切除术,观察组20例采用神经内镜下经鼻蝶切除术,比较两组患者的各项手术及恢复指标、并发症发生率、肿瘤全切除率及肿瘤复发率。结果观察组手术时间、术中出血量、住院时间明显低于对照组,并发症发生率明显低于对照组,肿瘤全切除率明显高于对照组,而肿瘤复发率明显低于对照组(P<0.05)。结论神经内镜下经鼻蝶切除术对垂体瘤的临床疗效更好,肿瘤全切除率高,并发症发生率低,术后恢复快,复发率低。Objective To compare the effects of neuroendoscopic transsphenoidal resection and microscopical resection in the treatment of hypophysoma.Methods Fifty patients with hypophysoma admitted to the Neurosurgery Department of our hospital from June 2014 to June 2018 were selected and were randomly divided into the control group(30 cases)and observation group(20 cases).The control group(30 cases)underwent the microscope resection,while the observation group underwent transsphenoidal resection under a neuroendoscopy.The indexes of surgery and recovery,complication rates,total resection of tumor and tumor recurrence were compared between the two groups.Results The operation time,intraoperative blood loss and hospital stay in the observation group were significantly lower than those in the control group,the incidence of complications was significantly lower than those in the control group,the total tumor removal rate was significantly higher than that in the control group,and the recurrence rate of tumor was significantly lower than that in the control group(P<0.05).Conclusion Neuroendoscopic transsphenoidal resection has a better clinical effect on hypophysoma,with a high total tumor resection rate,a low complication rate,a rapid postoperative recovery and a low recurrence rate.
