改良Hodge试验对亚胺培南耐药的肠杆菌科细菌的临床应用评价

摘　　要：目的 评价和分析改良Hodge试验对亚胺培南耐药的肠杆菌科细菌检验方法的特异性与敏感性,为肠杆菌科细菌亚胺培南耐药提供依据。方法 选取贵州省沿河土家族自治县人民医院2015年1月至2017年12月ICU病房和呼吸内科分离出的耐亚胺培南的大肠埃希菌26例和肺炎克雷伯菌22例,采用改良Hodge试验检测分析。结果 26例大肠埃希菌中有24例改良Hodge试验阳性,阳性率为92.3%,22例肺炎克雷伯菌中有21例改良Hodge试验阳性,阳性率为95.5%。结论 耐亚胺南的大肠埃希菌和肺炎克雷伯菌对改良Hodge试验呈较高的诊断阳性率。因此,采用简便实用的改良Hodge试验方法检测和分析耐亚胺培南肠杆菌科细菌的耐药机制,是一种较为实用的检验方法。Objective To evaluate and analyze the specificity and sensitivity of the modified Hodge test for the detection of imipenem resistant Enterobacteriaceae,so as to provide evidence for the resistance of Enterobacteriaceae to imipenem.Methods From January 2015 to December 2017,26 cases of imipenem resistant Escherichia coli and 22 cases of Klebsiella pneumoniae isolated from ICU ward and respiratory department were analyzed by modified Hodge test.Results In 26 cases of Escherichia coli,24 cases were positive for the modified Hodge test,the positive rate was 92.3%;among 22 cases of Klebsiella pneumoniae,21 cases were positive in Hodge test,the positive rate was 95.5%.Conclusion Escherichia coli and Klebsiella pneumoniae were highly specific and sensitive to the improved Hodge test.Therefore,it is a practical test method to detect and analyze the resistance mechanism of imipenem Enterobacteriaceae by simple and practical modified Hodge test.
