摘　　要：目的探讨影响胃癌卵巢转移瘤术后生存的因素。方法回顾性分析2002年1月至2018年5月在新疆医科大学附属肿瘤医院行胃癌卵巢转移瘤手术治疗的29例患者临床资料,对可能影响其预后的临床病理因素进行单因素及多因素分析。结果经29例胃癌卵巢转移瘤中位生存期为18.23月(4.37~49.15),全组患者的1、2、3年生存率分别为82.8%、37.9%及10.3%。单因素分析结果显示,肿瘤病理类型,转移部位数及腹膜转移是影响患者预后的影响因素(P<0.05),多因素分析结果显示,转移部位数及腹膜转移是影响胃癌卵巢转移瘤术后患者预后的独立危险因素(P<0.05)。结论对胃癌卵巢转移患者需要充分评估,行转移瘤切除术可能为此类患者带来生存期延长和生活质量提高。Objective To explore the clinicopathalogical features,treatment methods and other factors influencing the survival of patients with ovarian metastases from gastric cancer after operation.Methods Clinicopathalogical data of 29 gastric cancer patients with ovarian metastasis after operation between January 2002 and May 2018 of Tumor Hospital Affiliated to Xinjiang Medical University were analyzed retrospectively.Univariate and multivariate Cox regression were done to investigate the prognostic risk factors of these patients.Results The median overall survival was 18.23 months(4.37~49.15 months)of 29 gastric cancer patients with ovarian metastasis.The survival rates of 1-,2-,and 3 year were 82.8%,37.9%,and 10.3%,respectively.Univariate analysis showed that histological type,number of metastatic sites,and peritoneal metastasis were associated with prognosis(P<0.05).Multivariate analysis revealed that number of metastatic sites,and peritoneal metastasis were independent prognostic factors(P<0.05).Conclusion Patients with ovarian metastasis from gastric cancer need to be fully assessed.Resection of metastatic tumors may lead to prolonged survival and improved quality of life in these patients.
