摘 要:目的探讨超声对胎儿唇腭裂分度分型的临床诊断准确率。方法选取2016年5月~2018年9月期间于本院行产前超声筛查的1056例孕产妇,对全部胎儿均行二维超声检查及二维联合三维超声检查,随访两组胎儿临床结局,将两组临床诊断准确率、唇腭裂分型、分度及漏诊、误诊率作为观察指标进行对比。结果通过随访两组胎儿临床结局,产后发现唇腭裂19例,其中单纯性唇裂12例、唇腭裂7例。二维超声组诊断准确率为84.21%,二维联合三维超声组诊断准确率达到100.00%,P<0.05;二维超声组胎儿唇腭裂左侧6例,占37.50%;右侧7例,占43.75%;双侧3例,占18.75%;二维联合三维超声组唇腭裂左侧7例,占36.84%;右侧8例,占42.11%;双侧4例,占21.05%,P>0.05;二维超声组唇腭裂分度I度8例,占50.00%;II度7例,占43.75%;III度1例,占6.25%;二维联合三维超声组唇腭裂分度I度8例,占42.11%;II度8例,占42.11%;III度3例,占15.79%。两组唇腭裂分度III度比较差异具有统计学意义,P<0.05;二维超声组误诊2例,占10.53%;漏诊1例,占5.26%;二维联合三维超声组无误诊、漏诊例数,P<0.05。结论二维超声联合三维超声检查应用于胎儿唇腭裂产前筛查中具有重要临床价值,可进一步提高临床诊断准确率,减少误诊、漏诊情况发生,为临床医师选择分娩方式提供重要参考依据。Objective:To investigate the accuracy of ultrasound in the classification of fetal cleft lip and palate.Methods:From May 2016 to September 2018,1056 pregnant and lying-in women who underwent prenatal ultrasound screening in our hospital were selected.All fetuses were examined by two-dimensional ultrasound and two-dimensional combined three-dimensional ultrasound.The clinical outcomes of the two groups were followed up.The accuracy of clinical diagnosis,classification of cleft lip and palate,the rate of missed diagnosis and misdiagnosis were compared between the two groups.Results:The clinical outcomes of the two groups were followed up,19 cases of cleft lip and palate were found postpartum,included 12 cases of simple cleft lip and 7 cases of cleft lip and palate.The diagnostic accuracy of two-dimensional ultrasound group was 84.21%,and that of twodimensional combined three-dimensional ultrasound group was 100.00%,P<0.05.In the two-dimensional ultrasound group,there were 6 fetuses with cleft lip and palate on the left side,accounted for 37.50%.7 cases(43.75%)were on the right side.Bilateral in 3 cases(18.75%);In the two-dimensional combined three-dimensional ultrasound group,7 cases(36.84%)had cleft lip and palate on the left side.8 cases(42.11%)were on the right side.Bilateral 4 cases,accounted for 21.05%,P>0.05;In the twodimensional ultrasound group,8 cases(50.00%)had cleft lip and palate grade I.Seven cases(43.75%)were grade II.One case(6.25%)had degree III.In the two-dimensional combined three-dimensional ultrasound group,8 cases(42.11%)had cleft lip and palate grading I.8 cases(42.11%)had grade II.There were 3 cases of III degree,accounted for 15.79%.There was significant difference in the degree III of cleft lip and palate between the two groups(P<0.05).Two cases(10.53%)were misdiagnosed in the two-dimensional ultrasound group.One case was missed diagnosis,accounted for 5.26%.There were no cases of misdiagnosis or missed diagnosis in two-dimensional combined three-dimensional ultrasound group(P<0.05).Conclus
