单根管患牙根管治疗术中不同封药的疗效对比

摘　　要：目的比较3种根管封药糊剂作为根管消毒药物在单根管患牙二次根管治疗中的疗效。方法纳入合肥市第一人民医院口腔科2018年1月至6月因慢性根尖周炎接受根管治疗的90例患者(90例患牙)随机分为3组,在常规进行冠向下根管预备后分别使用Apexcal糊剂(Apexcal组)、通用型氢氧化钙糊剂(氢氧化钙组)和氯已定凝胶(氯已定组)进行根管封药1周,观察封药后3种药物的临床疗效以及对粪肠球菌的抑菌效果,并进行统计学分析。结果3组药物均能不同程度改善患者临床症状,减少粪肠球菌的感染,与封药前比较差异均有统计学意义(P<0.05),其中Apexcal组临床疗效明显优于氢氧化钙组和氯已定组(P<0.05),但氯已定组对粪肠球菌的抑菌效果最佳,差异有统计学意义(P<0.05)。结论与手调式氢氧化钙糊剂相比,Apexcal糊剂作为根管封药在改善临床症状方面更具优势,氯已定凝胶可以更好地抑制粪肠球菌。Objective To evaluate in vivo the antibacterial efficacy of three intracanal medicaments against Enterococcus faecaon in singal root canal during two-visit root cancal treatment.Methods A total of 90 patients(90 affected teeths)with chronic periapical periodontitis in stomatology department of Hefei first people's hospital from January to June 2018 were randomly divided into 3 groups.A first microbio-logical sample was obtained after regular canal preparation,and the root canals were filled with Apexcal paste as the Apexcal group,the root canals were filled with calcium hydroxide(CH)and chlorhexidine gel(CHX)respectively as CH group and CHX group.After a week,the dressing was removed and the second microbiological sample was collected.Enterococcus faecaon was assessed by real-time quantitative polymerase chain reaction before and after disinfection.The effects of root canal disinfection of 3 groups were recorded and analyzed.Results Three intracanal medicaments had improved the clinical symptoms and reduced the enterococcus faecalis infection,compared with that before treatment,the differences were significant(P<0.05).The clinical curative effects of Apexcal group was better than those of other groups(P<0.05),the chlorhexidine gel provided a more significant reduction in the number of Enterococcus faecaon than other groups,the differences were significant(P<0.05).Conclusion Compared with calcium hydroxide,Apexcal has an advantage as a intracanal medicament in improving root canal disinfectant periodontitis,and the chlorhexidine gel can efficiently inhibit Enterococcus faecaons.
