理气养胃汤联合常规西药治疗肝胃气滞型慢性糜烂性胃炎临床研究

摘　　要：目的:观察理气养胃汤治疗肝胃气滞型慢性糜烂性胃炎(Chronic erosive gastritis,CEG)的疗效及其对血清胃蛋白酶原(Pepsinogen,PG)含量和焦虑抑郁状态的影响。方法:把经胃镜检查提示为CEG且中医辨证为肝胃气滞的患者共95例随机分为观察组和对照组,对照组47例给予常规西药治疗,观察组48例在对照组基础上加用理气养胃汤进行治疗。2组患者疗程均为2周,观察治疗前后中医症状积分、胃镜检查结果、PGⅠ、PGⅡ数值变化、汉密尔顿焦虑量表(Hamilton anxiety scale,HAMA)评分和汉密尔顿抑郁量表(Hamilton depression scale,HAMD)评分变化。结果:通过2组患者治疗后临床症状积分及胃镜检查结果对比,观察组治疗效果明显优于对照组(P<0.05)。与治疗前比较,2组患者治疗后血清PGⅠ、PGⅡ数值均有下降(P<0.01,P<0.05);与对照组比较,观察组血清PGI、PGII数值下降更明显(P<0.05);2组患者HAMA和HAMD评分均明显下降(P<0.01,P<0.05);与对照组比较,观察组下降更明显(P<0.05)。结论:理气养胃汤联合常规西药治疗肝胃气滞型CEG疗效显著,中医症状疗效和胃镜检查疗效均较理想,且能降低血清PGⅠ、PGⅡ水平,同时能有效改善焦虑抑郁状态。Objective:To observe the curative effect of Liqi Yangwei tang for chronic erosive gastritis(CEG) of stagnation of liver-qi and stomach-qi type and its effect on the content of pepsinogen(PG) in serum as well as anxiety and depression.Methods:A total of 95 cases of patients who were diagnosed as CEG by gastroscopy and stagnation of liver-qi and stomach-qi through syndrome differentiation were randomly divided into the observation group and the control group,48 and47 cases in each group respectively.The control group was treated with routine western medicine,and the observation group was additionally treated with Liqi Yangwei tang based on the treatment of the control group.Both groups were treated for two weeks.Before and after treatment,the changes in the scores of Chinese medicine symptom,the results of gastroscopy,the values of PGI and PGII,and the scores of Hamilton anxiety scale(HAMA) as well as Hamilton depression scale(HAMD)were observed.Results:After treatment,compared the scores of clinical symptoms and the results of gastroscopy in the two groups,the therapeutic effect in the observation group was superior to that in the control group(P<0.05).After treatment,the values of PGI and PGII in serum in the two groups were decreased when compared with those before treatment(P<0.01,P<0.05);the decrease of the above values in the observation group was more obvious when compared with those in the control group(P<0.05);the scores of HAMA and HAMD in the two groups were significantly decreased(P<0.01,P<0.05);the decrease of the above scores in the observation group was more obvious when compared with that in the control group(P<0.05).Conclusion:The therapy of Liqi Yangwei tang combined with routine western medicine has significant curative effect in treating CEG of stagnation of liver-qi and stomach-qi type with ideal curative effect on both Chinese medicine symptoms and gastroscopy,which can reduce the levels of PGI and PGII in serum,and effectively alleviate anxiety and depression.
