基于表面肌电研究互动式针法深刺廉泉对脑卒中后流涎患者的影响

摘　　要：目的:观察互动式针法深刺廉泉治疗脑卒中后流涎的临床疗效。方法:观察病例为2015年6月-2018年6月本院住院及门诊的脑卒中后流涎患者,共60例。按随机数字表法分为2组各30例,对照组给予常规吞咽训练,治疗组在对照组常规吞咽训练方案的基础上采用深刺廉泉的同时结合患者主动吞咽口水动作治疗。在治疗开始前及治疗4周后分别采用教师流涎分级法(TDS)及表面肌电图进行观察评价。结果:治疗后,2组颏下肌群静息状态及吞咽状态表面肌电信号分别比较,差异均有统计学意义(P<0.05)。2组TDS分级情况比较,差异有统计学意义(P<0.05)。治疗组优于对照组。结论:互动式针法可改善脑卒中后流涎症状,疗效优于单纯采用常规吞咽训练治疗。Objective:To observe the clinical effect of complementary dynamic acupuncture therapy with deep needling at Lianquan(CV23) for sialorrhea after stroke.Methods:A total of 60 cases of inpatients and outpatients with sialorrhea after stroke in our hospital from June 2015 to June 2018 were observed and divided into two groups according to the random number table method,30 cases in each group.The control group was given routine swallowing exercises;the treatment group was additionally treated with deep needling at Lianquan(CV23) based on the treatment of the control group,and combined with active swallowing saliva at the same time.Before treatment and four weeks after treatment,Teacher Drooling Scale(TDS) and surface electromyography were applied in observation and evaluation respectively.Results:After treatment,the respective comparison of the resting state of submental muscle group and surface electromyography signals in swallowing state in the two groups showed significance in differences(P<0.05).The comparison of clinical effect with TDS in the two groups showed significance in the difference(P<0.05),and the one in the treatment group was superior to that in the control group.Conclusion:Complementary dynamic acupuncture therapy can improve sialorrhea after stroke,whose curative effect is superior to that of simple routine swallowing exercises.
