内镜下套扎术辅助内镜黏膜下剥离术治疗胃黏膜下肿瘤临床研究

摘　　要：目的研究内镜下套扎术辅助内镜黏膜下剥离术治疗胃黏膜下肿瘤临床效果。方法选取2015年9月至2018年1月在本院接受治疗的58例胃黏膜下肿瘤患者,随机分为对照组和研究组,各29例。对照组接受内镜黏膜下剥离术治疗,研究组接受内镜黏膜下剥离术联合内镜下套扎术治疗。比较两组住院时间、治疗费用、肿瘤切除率及并发症发生率。结果研究组住院时间、治疗费用分别为(7.58±1.54)d、(9 122.98±1 054.21)元,均明显低于对照组(10.36±2.28)d、(9 874.63±1 421.36)元,差异均有统计学意义(P<0.05);研究组的术中出血、感染、术中穿孔的发生率均明显低于对照组,而肿瘤完整切除率明显高于对照组,差异均有统计学意义(P<0.05)。结论对胃黏膜下肿瘤患者应用镜下套扎术辅助内镜黏膜下剥离术治疗,可降低并发症的发生,提高肿瘤切除率,降低医疗费用,临床优势明显。Objective To study the clinical effect of gastric submucosal tumor treated by Endoscopic Variceal Ligation assisting Endoscopic Submucosal Dissection. Methods 58 selected cases of patients with gastric submucosal tumor in our hospital from the September 2015 to January2018 were randomly divided into the control group and treatment group, with 29 cases in each group. The control group received the endoscopic submucosal dissection, the treatment group received the endoscopic submucosal dissection combined with endoscopic variceal ligation. The hospital stays, treatment charge, resection rate of tumor and the incidence of complications of two groups were compared. Results The hospital stays and treatment charge of treatment group were(7.58±1.54) d and(9 122.98±1 054.21) yuan, which were significantly lower than the(10.36±2.28) d and(9 874.63±1 421.36) yuan of the control group, there was statistical significance(P<0.05). The occurrence rate of the hemorrhage during operation,infection and intraoperative perforation of the treatment group were significantly lower than the control group, but the tumour complete excision rate was significantly greater than the control group, there was statistical significance(P<0.05). Conclusion Endoscopic Variceal Ligation assisting Endoscopic Submucosal Dissection for treating patients with gastric submucosal tumor can reduce the occurrence of complications, improve the resection rate of tumor, decrease the treatment charge and with obvious clinical advantages.
