摘　　要：目的探讨Lvis支架在后循环分叉部宽颈动脉瘤的应用及疗效。方法回顾性分析采用Lvis支架辅助弹簧圈栓塞治疗的15例后循环分叉部颅内宽颈动脉瘤病人的临床资料,其中蛛网膜下腔出血5例,Hunt-Hess分级:Ⅰ级2例,Ⅱ级2例,Ⅲ级1例。结果术后栓塞程度Raymond分级:Ⅰ级11例,Ⅱ级4例。术后死亡1例。随访14例,时间3~18个月,平均(7.5±1.3)个月,mRS评分:0~1分10例,2分3例,4分1例。动脉瘤造影复查显示:Ⅰ级栓塞的11例复查动脉瘤不显影;Ⅱ级栓塞3例中,改善1例,复发2例,需要再次治疗。结论Lvis支架在颅内后循环分叉部宽颈动脉瘤的应用短期疗效好,远期疗效需进一步观察。Objective To explore the application and outcome of Lvis stent in the treatment of wide-necked aneurysms in the furcation of the posterior circulation.Methods Clinical data of 15 patients with wide-necked aneurysms in the furcation of the posterior circulation were analyzed respectively,who received Lvis stent treatment assisted with coil embolism.There were 5 patients with subarachnoid hemorrhage and the Hunt-Hess grade was gradeⅠin 2 patients,gradeⅡin 2 and gradeⅢin 1.Results The embolism degree were Raymond gradeⅠin 11 patients and gradeⅡin 4.One patient died after surgery.Follow-up was performed on 14 patients for 3 to 18 months with a mean time of 7.5±1.3 months.The mRS results were 0 to 1 point in 10 patients,2 points in 3 and 4 points in 1.No development was observed in 11 patients with embolism degree of Raymond gradeⅠ.In the 3 patients with embolism degree of Raymond gradeⅡ,improvement occurred in 1 and relapse in 2,who needed the second treatment.Conclusion Application of Lvis stent in the treatment of wide-necked aneurysms in the furcation of the posterior circulation can achieve good efficacy in short term,but the long-term outcome needs further observation.
