摘　　要：随着物质生活水平不断提高和老龄化进程加剧,慢性病已经成为影响我国居民身心健康的重要疾病,给社会和家庭带来沉重的疾病负担。文章以客户关系管理理论为基础,从对象、供给者、内容、方式等四个要素构建社区慢性病健康教育的新模式,并提出完善健康信息系统、整合社区卫生健康资源、建立反馈和效果评价机制等实施路径。With the continuous improvement of material living standards and the aggravation of the aging process,chronic diseases have become an important disease affecting the physical and mental health of Chinese residents,bringing heavy disease burden to society and families.Based on the theory of customer relationship management,this paper constructs a new model of community health education for chronic diseases from four elements:object,provider,content and method,and puts forward the implementation path of improving health information system,integrating community health resources,establishing feedback and effect evaluation mechanism.
