颊脂垫转移在老年人颊粘膜缺损治疗中的临床价值

摘　　要：目的探究颊脂垫转移在老年人颊粘膜缺损治疗中的临床应用价值。方法选取2017年3月至2019年3月我院收治的120例老年颊粘膜缺损患者作为本次研究对象,所选患者采用带蒂颊脂垫组织修复颊粘膜缺损,分析其治疗效果。结果对患者进行6个月的随访工作,术后患者口腔转移的颊脂垫均出现水肿现象,120例患者中,2例出现感染,7 d后67例患者水肿症状有所减轻,14 d后所有患者的颊脂垫水肿明显改善,并且颊脂垫外膜出现上皮化现象;在手术后的5周,患者转移的颊脂垫上皮化更为显著,8周后,118例患者的颊脂垫完全上皮化,其颜色与正常粘膜组织相近;术后6个月,117例患者颊脂垫转移成功,进食和语言功能未受到较大影响,2例患者肿瘤原位复发,1例患者颊脂垫术后感染,颊脂垫转移组织出现坏死。结论颊脂垫转移治疗老年颊粘膜缺损其手术简单,效果理想,术后患者恢复较快,较为美观,是修复颊粘膜缺损的良好手段,值得推广应用。Objective To explore the clinical value of buccal fat pad transfer in the treatment of buccal mucosal defect in the elderly.Methods One hundred and twenty elderly patients with buccal mucosal defect admitted to our hospital from March 2017 to March 2019 were selected as the research object.The patients were treated with pedicled buccal fat pad to repair buccal mucosal defect,and the therapeutic effect was analyzed.Results The patients were followed up for 6 months.There were edema in buccal fat pads with oral metastasis after operation.Among 120 patients,2 had infection.After 7 days,edema symptoms were alleviated in 67 patients.After 14 days,edema of buccal fat pads was improved in all patients,and epithelialization of the adventitia of buccal fat pads occurred.At 5 weeks after operation,the epithelialization of buccal fat pads with oral metastasis was more significant.Six months after operation,117 patients succeeded in metastasis of the buccal fat pad,with no significant impact on their eating and language function.Two patients had recurrence in situ.One patient had infection after operation and necrosis of the metastatic tissue of the buccal fat pad.Conclusion Buccal fat pad transfer for the treatment of buccal mucosal defect in the elderly is simple and effective.The patients recover faster and more beautiful after operation.It is a good method for repairing buccal mucosal defect.It is worth popularizing and applying.
