摘　　要：目的观察自体富血小板纤维蛋白凝胶(PRF)在颌骨中央型骨髓炎致骨缺损骨修复中的临床效果。方法将2014年6月~2016年12月就诊于南充市中心医院口腔科的中央型颌骨骨髓炎60例患者随机分为观察组和对照组,每组各30例。观察组患者行颌骨骨髓炎死骨病灶清除术同期行PRF凝胶+止血海绵填塞骨腔;对照组患者行颌骨骨髓炎死骨清除术后仅填塞止血海绵。术后1、3、6个月行CBCT检查并用医学图像分析软件EasyDen及Ez3d plus进行骨腔骨密度(HU)值测量。结果术后1个月,两组骨缺损区的HU值比较差异无明显统计学意义(P>0.05);术后3个月,两组比较差异有统计学意义(P<0.05);术后6个月两组骨缺损区的骨密度值比较差异有统计学意义(P<0.05)。术后6个月观察组骨缺损区的骨密度已经趋于正常的颌骨骨密度。结论 PRF凝胶可明显诱导和促进颌骨中央型骨髓炎致骨缺损骨组织的形成和修复,为颌骨中央型骨髓炎患者骨缺损修复提供一种可选择的临床应用方案。Objective To observe the clinical effect that autologous platelet fibrin rich(platelet-rich fibrin, PRF) in bone defect repair of jaw central osteomyelitis. Methods From June 2014 to December 2016,120 jaw central osteomyelitis patients in Stomatological Department of Nanchong central hospital were randomly divided into observation group and control group, The observation group(30 patients) chosen the operation plan of PRF composite with absorbable gelatin sponge. The control group(30 cases) chosen implant absorbable gelatin sponge. All patients were informed consent and voluntary choose surgical plan. Generally, postoperative bone density was measured in 1 month, 3 month, 6 month after CBCT radiography examination. EasyDent and Ez3 d plus analysis were used to measure the two groups bone density(HU).Results 1 month after operation, HU of experimental group and the control group were(290.58±13.20)and(284.05±12.29)(t=1.78,P>0.05). 3 month after operation, HU of experimental group and the control group were(377.73±23.56)and(298.12±14.48)(t=17.11,P<0.05). 6 month after operation, HU of observation group and the control group were(489.11±10.76)and(383.70±19.17)(t=25.07,P<0.05). Six months after the operation, the bone density in the bone defect area of the observation group had tended to normal jaw bone density.Conclusion PRF can obviously induce and promote the formation and repair of bone tissue caused by jaw central osteomyelitis, providing an alternative clinical application scheme for the repair of bone defects in patients with jaw central osteomyelitis.
