探讨原发性卵巢平滑肌瘤的影像学诊断价值

摘　　要：目的探讨原发性卵巢平滑肌瘤(PLO)的临床与影像学特点,评价影像学检查对其的诊断价值.方法回顾性分析8例经手术病理证实PLO患者的临床及超声、CT或MRI的影像学资料.结果7例患者均于绝经前发现;实验室检查仅1例患者血清CA125异常,其余血清相关肿瘤指标均正常.超声检查肿块均为实性低回声,边界清晰;彩色多普勒示6例中等及以上血流信号,其中5例阻力指数(RI)平均0.50;其余2例无明显血流信号;2例行CT检查肿块均呈低密度,边界清晰,增强扫描均明显强化,强化程度略低于子宫肌层;6例行MRI检查:其中5例肿块TlWI等信号,T2WI低信号,增强扫描均为不均匀强化,强化程度与子宫肌层相似.结论PLO影像学表现与子宫肌瘤类似,影像学检查对其诊断具有一定的价值.Objective To investigate the clinical and imaging features of primary leiomyoma of ovary(PLO)and evaluate the diagnostic value of imaging examination.Methods A retrospective analysis of 8 cases of PLO patients with clinical and ultrasound,CT or MRI imaging data confirmed by surgery and pathology.Results 7 of all patients were found before menopause.Only one patient had abnormal serum CA125 in the laboratory,and the other serum-related tumor indicators were normal.Ultrasound examination of the mass were solid hypoechoic,clear boundary color Doppler showed 6 cases of moderate and above blood flow signals,of which 5 cases of resistance index(RI)averaged 0.50.The remaining 2 cases had no obvious blood flow signal.2 cases CT examination showed that the mass had low density and clear boundary.CT enhanced scans were significantly enhanced,and the degree of enhancement was slightly lower than that of the myometriu m.Six cases underwent MRI examination.Five of them had TIWI iso-signal and T2W1 hypo-signal.The enhancement degree was similar to that of uterine myometrium.Conclusion PLO imaging features are similar to uterine fibroids,and imaging examination have certain value for their diagnosis.
