健脾益肺方联合西药治疗肺脾两虚型肌萎缩侧索硬化症临床研究

摘　　要：[目的]观察健脾益肺方联合西药对肺脾两虚型肌萎缩侧索硬化症(ALS)的治疗效果.[方法]纳入符合标准的患者30例,所有患者在对症支持治疗(包括营养支持、呼吸支持和利鲁唑口服治疗)的基础上,加用健脾益肺方中药汤剂(由党参、黄芪、白术、柴胡、杜仲、五味子、菟丝子、制马钱子、法半夏、麦冬、陈皮、杏仁、僵蚕、桔梗、炙甘草等组成),疗程为8周.观察治疗前、治疗8周后患者ALS功能量表(ALSFRS-R)总分和中医证候积分的变化情况,并评价其疗效.[结果](1)健脾益肺方联合西药治疗肺脾两虚型ALS患者的总有效率为56.7%,且患者在气短喘促、声短气怯、咳声低怯、痰、息弱5个方面的评分及中医证候总积分均较治疗前明显改善,差异均有统计学意义(P<0.05或P<0.01).(2)治疗后,患者ALSFRS-R总分较治疗前有所升高,但差异无统计学意义(P>0.05).[结论]健脾益肺方联合西药能明显改善肺脾两虚型ALS患者的中医证候及呼吸功能.Objective To observe the effect of Jianpi Yifei Recipe combined with western medicine for the treatment of amyotrophic lateral sclerosis(ALS)with lung and spleen deficiency type.Methods A total of 30 cases of ALS patients with lung and spleen deficiency type were treated with supportive and symptomatic treatment(nutritional support therapy,respiratory support with mechanical ventilation,and oral use of Riruzole)plus Jianpi Yifei Recipe(mainly composed of Radix Codonopsis,Radix Astragali,Rhizoma Atractylodis Macrocephalae,Radix Bupleuri,Cortex Eucommiae,Fructus Schisandrae Chinensis,Semen Cuscutae,Semen Strychni Preparata,Rhizoma Pinelliae Preparata,Radix Ophiopogonis,Pericarpium Citri Reticulatae,Semen Armeniacae Amarum,Bombyx Batryticatus,Radix Platycodonis,and Radix Glycirrhizae Preparata).The treatment course covered 8 weeks.Changes of the Amyotrophic Lateral Sclerosis Functional Rating Scalerevised(ALSFRS-R)overall scores,traditional Chinese medicine(TCM)symptom scores and clinical efficacy were observed before intervention and after intervention for 8 weeks,and the clinical efficacy was evaluated.Results(1)After intervention with Jianpi Yifei Recipe combined with western medicine,the total effective rate for the 30 cases of ALS was 56.7%,and the scores for shortness of breath,asthma,low and timorous voice,cough with low and faint voice,expectoration,weak breath as well as the overall scores of TCM symptom were much improved(P<0.05 or P<0.01 compared with those before intervention).(2)After intervention,ALSFRS-R overall scores were mildly increased,but the difference was insignificant(P>0.05 compared with those before intervention).Conclusion Jianpi Yifei Recipe combined with western medicine can improve the respiratory function and TCM symptoms of ALS patients with lung and spleen deficiency type.
