摘  要:目的探讨理气化痰活血方对冠状动脉微循环障碍的影响及可能作用机制。方法健康雄性SD大鼠245只随机分为理气化痰活血方组、缬沙坦组、模型组各80只,空白组5只。除空白组外,其余3组均采用经心尖部向左心室注射月桂酸钠的方法建立大鼠冠状动脉微循环障碍模型。理气化痰活血方组及缬沙坦组术前1天及术后4周分别给予理气化痰活血方药(浓度2.25 g/ml)、缬沙坦悬浊液(浓度8.89 mg/ml)1 ml/100 g灌胃,模型组及空白组给予1 ml/100 g蒸馏水灌胃,均每日1次。实验期间观察各组大鼠死亡情况;理气化痰活血方组、缬沙坦组、模型组分别于术后1 h、24 h、1周、2周、3周、4周各时间点随机处死5只大鼠,空白组4周时处死,检测各组大鼠血清内皮素1(ET-1)、一氧化氮(NO)、血管性血友病因子(vWF)浓度及心肌组织纤维介素(fgl 2)mRNA表达。结果模型组、理气化痰活血方组、缬沙坦组大鼠总计分别死亡44只、40只、42只,3组大鼠术中及术后死亡情况比较差异无统计学意义(P>0.05)。术后4周时,模型组大鼠较空白组血清ET-1浓度升高,vWF浓度降低(P<0.01),血清NO浓度和心肌组织fgl 2 mRNA表达差异无统计学意义(P>0.05)。与模型组同时间比较,理气化痰活血方组和缬沙坦组术后1 h、24 h、1周、2周、3周、4周血清ET-1浓度降低(P<0.05或P<0.01)。与模型组和缬沙坦组同时间比较,术后4周理气化痰活血方组血清NO浓度升高,vWF浓度降低(P<0.05或P<0.01)。术后2周、3周理气化痰活血方组心肌组织fgl 2 mRNA表达显著低于模型组(P<0.01)。结论理气化痰活血方可能通过降低冠状动脉微循环障碍大鼠血清ET-1、vWF浓度及心肌组织fgl 2 mRNA表达,升高血清NO浓度,从而起到保护血管内皮功能的作用。Objective To explore the possible mechanism of action of Liqi Huatan Huoxue Formula(理气化痰活血方)in improving coronary microcirculation disturbance.Methods A total of 245 healthy male SD rats were randomly divided into a Liqi Huatan Huoxue Formula group,a valsartan group and a model group,with 80 rats in each group.In addition,a blank group was set with 5 rats.Except for the blank group,the other 3 groups were established a model of coronary microcirculation disorder by injecting sodium laurate into the left ventricle.The Liqi Huatan Huoxue Formula group and the valsartan group were given Liqi Huatan Huoxue Formula(concentration 2.25 g/ml)and valsartan suspension(concentration 8.89 mg/ml)1 ml/100 g 1 day before operation and 4 weeks after operation.The model group and the blank group were given 1 ml/100 g of distilled water for gavage,once a day.During the experiment,the deaths of the rats in each group were observed.Five rats were randomly sacrificed at the time points of 1 h,24 h,1 w,2 w,3 w and 4 w after the operation in the Liqi Huatan Huoxue Formula group,the valsartan group and the model group.The rats were sacrificed at 4 w in the blank group.The concentrations of serum endothelin-1(ET-1),nitric oxide(NO),von Willebrand factor(vWF)and myocardial tissue fibrin(fgl 2)mRNA were detected.Results There were 44 deaths,40 deaths,and 42 deaths respectively in the model group,Liqi Huatan Huoxue Formula group and the valsartan group.There was no statistically significant difference in the intraoperative and postoperative mortality among the 3 groups(P>0.05).At 4 w after operation,the concentration of serum ET-1 in the model group was higher than that in the blank group,and the concentration of vWF was decreased(P<0.01).There was no significant difference in serum NO concentration and fgl 2 mRNA expression between the 2 groups(P>0.05).At the same time,compared with the model group,the serum ET-1 concentrations in the Liqi Huatan Huoxue Formula group and the valsartan group were decreased at 1 h,24 h,1 w,2 w,
