摘 要:目的探讨一种应用负压细针穿刺细胞(FNAC)诊断甲状腺疾病快而稳定的实验程序。方法对吉林大学中日联谊医院病理科1200例甲状腺疾病患者的临床病理诊断进行分析,对术前FNAC技术和术后石蜡实验技术的诊断结果进行比较分析。结果1200例术后诊断甲状腺癌827例(68.9%),甲状腺腺瘤267例(22.2%),甲状腺囊肿66例(5.5%),甲状腺功能亢进31例(2.6%),甲状腺炎9例(0.8%)。FNAC细胞诊断和术后石蜡标准诊断比较,良性病变诊断百分比分别为28.4%和31.1%,恶性病变诊断百分比分别为65.5%和68.9%,FNAC细胞诊断良、恶性诊断效能百分比分别为91.3%和95.1%,良、恶性病变诊断的特异性百分率比较,差异无统计学意义(χ2=7.145,P>0.05)。结论FNAC方法诊断甲状腺疾病可靠、方便快捷、经济适用,也可用于甲状腺手术的术前诊断。Objective To explore an economical and effective method for the diagnosis of thyroid diseases by fine needle aspiration cytology(FNAC).Methods The clinicopathological diagnosis of 1200 cases of thyroid diseases in our department was analyzed.Results There were 827 cases(68.9%)of thyroid carcinoma,267 cases(22.2%)of thyroid adenoma,66 cases(5.5%)of thyroid cyst,31 cases(2.6%)of hyperthyroidism,and 9 cases(0.8%)of thyroiditis.FNAC cell diagnosis and postoperative paraffin standard comparison,benign lesions diagnosed percentage were 28.4%and 31.1%respectively,Diagnosis of malignant change percentage were 65.5%and 68.9%respectively,FNAC diagnosis of benign and malignant diagnosis efficiency cells percentage were 91.3%and 95.1%respectively,The specificity of the diagnosis of benign and malignant lesions compared percentage,there was no statistically significant difference(χ2=7.145,P>0.05).Conclusion FNAC method is reliable,convenient and economical in diagnosing thyroid diseases.It can also be used for preoperative diagnosis of thyroid surgery.
