生物胶联合邻位肌瓣在复杂乳糜瘘治疗中的应用

摘　　要：目的:探讨分析医用生物胶联合邻位肌瓣治疗颈淋巴清扫术后复杂乳糜瘘的临床可行性和有效性。方法:选取2016年6月~2018年6月经颈淋巴清扫术后复杂乳糜瘘患者5例,其经保守治疗均无效,二次手术探查发现胸导管瘘口>5 mm,无法常规缝扎或结扎胸导管。应用医用生物胶联合邻位肌瓣治疗复杂乳糜瘘患者,并观察其疗效。结果:5例患者术后均未再次出现乳糜瘘及其它严重并发症。术后随访5例患者术后痊愈,术区无不适,颈部贴附良好。3个月内均未发现乳糜瘘复发,未出现局部不良反应。结论:应用医用生物胶联合邻位肌瓣是治疗颈淋巴清扫术后复杂乳糜瘘一种安全、高效的方法,值得临床推广。Objective:To explore the feasibility and effective value of medical adhesive glue combined with regional muscle flap in the treatment of complicated chylous leakage after cervical lymph dissection.Methods:A series of 5 patients were enrolled in this study from June 2016 to June 2018.All patients accepted medical adhesive glue combined with regional muscle flap for treatment of complicated chylous leakage.The secondary surgical exploration showed that the fistulas of thoracic duct were more than 5 mm in diameter,and it was difficult for treatment of this kind of fistulas by suturing the thoracic duct.Results:There was no recurrence of chylous leakage or other serious complications.No local adverse reactions,such as allergic reactions to the COMPONT medical gel and infection,were observed within 3 months.All patients in this study were cured finally with no discomfort and recurrence of chylous leakage after 3 months follow-up.Conclusion:It is a safe and reliable method for treatment of complicated chylous leakage by using COMPONT medical gel combined with regional muscle flap.
