摘  要:目的:运用Meta分析法评价还原型谷胱甘肽预防化疗所致神经毒性的疗效与安全性,为临床提供循证依据。方法:检索Cochrane library、Pubmed、Ovid、中国期刊全文数据库、万方资源数据库、维普数据库和中国生物医学文献数据库系统,对纳入的随机对照试验(RCT),根据改良的Jadad量表进行文献质量评价,并运用RevMan 5.3软件进行统计学分析。结果:共纳入19项RCTs,1436例患者。Meta分析结果显示,还原型谷胱甘肽组总的神经毒性发生率、严重神经毒性发生率及累积神经毒性发生率均低于对照组。亚组分析显示,与高剂量还原型谷胱甘肽(3g/m^2)相比,低剂量的还原型谷胱甘肽(1.5 g/m^2)即可显著降低总神经毒性及严重神经毒性发生率。结论:低剂量的还原型谷胱甘肽可以有效预防化疗药物所致外周神经毒性的发生,值得临床应用。Objective:To evaluate the efficacy and safety of glutathione in preventing peripheral neurotoxicity of chemotherapeutic drugs by Meta-analysis,and to provide evidence-based reference for clinical practice.Methods:The Cochrane Library,Pubmed,Ovid,CNKI,WanFang,VIP and CBM were retrieved by computer,and the quality assessment of qualified random control trials(RCT)was evaluated according to Jadad scale and statistic analysis was carried out by RevMan 5.3 software.Results:A total of nineteen RCTs and 1436 patients were included.Meta-analysis indicated that total incidence of peripheral neurotoxicity,incidence of severe neurotoxicity and cumulative neurotoxicity in GSH group were significantly lower than control group.Subgroup Meta-analysis indicated that compared with high dose of glutathione(3 g/m 2),lower dose of glutathione(1.5 g/m 2)could significantly reduce the incidence of total neurotoxicity and severe neurotoxicity.Conclusion:Lower dose of glutathione could effectively prevent peripheral neurotoxicity of chemotherapeutic drugs,and worthy to be popularized in clinical application.
