摘　　要：目的观察呼吸末正压Acapella Duet联合主动呼吸循环技术(ACBT)对脑卒中合并肺部感染患者肺功能的影响。方法选取2018年4月至2019年3月收治的30例脑卒中合并肺部感染患者,随机分为观察组与对照组各15例。对照组给予常规抗感染治疗及呼吸功能训练(含ACBT),观察组在对照组基础上加用Acapella Duet震荡排痰器。两组每日治疗2次,每次30min,比较两组患者治疗前后的排痰效果和肺功能。结果治疗3周后,两组患者的FVC、 FEV1、 PEF均提高(P <0.05),且观察组的FVC、 FEV1、 PEF优于对照组(P <0.05)。治疗后,观察组的咳嗽、咳痰、肺部湿啰音持续时间均短于对照组(P <0.05)。结论呼吸末正压Acapella Duet联合ACBT能够在短期显著提高患者的排痰效果,缩短其住院时间,改善通气障碍,对脑卒中后并发肺部感染的防治具有积极作用,值得推广应用。Objective To observe the influence of Acapella Duet and active cycle of breathing techniques(ACBT) on pulmonary function of patients with stroke complicated with pulmonary infection. Methods 30 patients with stroke complicated with pulmonary infection admitted to our hospital from April 2018 to March 2019 were selected and randomly divided into observation group and control group, with 15 cases in each group. The control group was given routine anti-infection therapy and respiratory function training(including ACBT), while the observation group used Acapella Duet concussive sputum evacuator on the basis of the control group. Both groups were treated twice a day for 30 minutes each time. The sputum evacuation effect and pulmonary function before and after treatment were compared between the two groups. Results 3 weeks after treatment, the FVC, FEV1 and PEF of both groups increased(P <0.05), and the FVC, FEV1 and PEF of the observation group were better than those of the control group(P <0.05). After treatment, the duration of cough,sputum and lung moist rale in the observation group were shorter than those in the control group(P <0.05). Conclusions Acapella Duet combined with ACBT can significantly improve the sputum evacuation effect in the short term, shorten the hospitalization time, and improve the ventilation disorder, with positive effect in the prevention and treatment of pulmonary infection after stroke, which is worthy of promotion and application.
