摘 要:目的探讨脂蛋白相关磷脂酶A2 (Lp-PLA2)水平与脑梗死患者血脂及颈动脉病变的关系。方法选取2018年1月至2018年12月我院收治的200例脑梗死患者作为试验组,另选取同期于我院200名健康体检者作为对照组。测定Lp-PLA2、HDL-C、 TC、 TG、 LDL-C水平及颈动脉病变。分析Lp-PLA2与血脂、颈动脉病变相关性。结果试验组的TC、 TG、 LDLC、 Lp-PLA2水平高于对照组(P <0.05)。两组的HDL-C水平比较无统计学差异(P>0.05)。四组中,不稳定斑块组的LpPLA2水平最高,其次是混合斑块组、稳定斑块组,无斑块组最低,比较差异有统计学意义(P <0.05)。经双变量Pearson相关性分析检验证实,Lp-PLA2水平与TG、 LDL-C、颈动脉病变呈正相关(r>0, P <0.05),与TC水平无明显相关(r>0, P>0.05)。结论 Lp-PLA2水平与血脂代谢、颈动脉病变有关,可反映颈动脉粥样硬化斑块情况,可作为预测脑梗死重要筛查指标。Objective To explore the correlation between the level of lipoprotein-associated phospholipase A2(Lp-PLA2) and blood lipid and carotid artery lesions in patients with cerebral infarction. Methods 200 patients with cerebral infarction admitted to our hospital from January 2018 to December 2018 were selected as experimental group, and 200 healthy people with health examination at the same period were selected as control group. The Lp-PLA2, HDL-C, TC, TG and LDL-C levels, and carotid artery lesions were detected. The correlation between Lp-PLA2 and blood lipid and carotid artery lesions was analyze. Results The TC, TG, LDL-C and Lp-PLA2 levels of the experimental group were higher than those of the control group(P <0.05). No statistical difference was found in the HDL-C level between the two groups(P >0.05). The Lp-PLA2 level in the unstable plaque group was the highest in the four groups, followed by the mixed plaque group and the stable plaque group, and the lowest in the non-plaque group(P <0.05). Bivariate Pearson correlation analysis confirmed that Lp-PLA2 level was positively correlated with TG, LDL-C and carotid artery lesions(r >0, P <0.05), but had no correlation with TC level(r >0, P >0.05). Conclusions Lp-PLA2 level is related to lipid metabolism and carotid artery lesions, which can reflect the carotid atherosclerosis plaque and can be used as an important screening index for predicting cerebral infarction.
