摘　　要：目的探讨消化道炎性纤维性息肉(IFP)的临床病理特点及治疗方法。方法回顾性分析我院2013年2月~2019年4月经内镜切除和普通外科手术切除的29例消化道IFP患者的临床表现和病理特征。结果 29例患者中肿块位于胃部24例,结肠1例,食管2例,小肠2例。大多数病变为单发(28例),少数为多发(1例)。肿块位于粘膜下层,可累及粘膜层,由短梭至梭形的间质细胞组成,病变内含薄壁小血管,梭形细胞常围绕血管排列,形成"洋葱皮"样结构,个别可伴有胶原化,间质疏松,常常伴有大量嗜酸性粒细胞浸润;部分病例间质可伴有粘液样变。肿瘤细胞表达Vimentin(100.00%),CD34(86.21%),部分病例可表达SMA(20.69%),不表达CD117,DOG-1,S-100。结论炎性纤维息肉是消化道少见的肿瘤,具有特征性的组织学改变及免疫组化,手术或内镜下可完全切除,预后良好。Objective To explore the clinicopathological features and treatment of inflammatory inflammatory fibrous polyps(IFP). Methods The clinical manifestations and pathological features of 29 patients with digestive tract IFP who underwent endoscopic resection and general surgery in our hospital from February 2013 to April 2019 were retrospectively analyzed. Results The mass was located in 24 cases of the stomach, 1 case of the colon, 2 cases of the esophagus, and 2 cases of the small intestine. Most lesions were single(28 cases) and a few were multiple(1 case). The mass is located in the submucosal layer, which may involve the mucosal layer. It consists of short-stem to fusiform interstitial cells. The lesion contains thinwalled small blood vessels. The spindle cells are often arranged around the blood vessels to form an "onion skin"-like structure, which may be accompanied by Collagen, interstitial loose, often accompanied by a large number of eosinophil infiltration;some cases of interstitial may be accompanied by mucus-like changes. Tumor cells express Vimentin(100.00%), CD34(86.21%), and some cases can express SMA(20.69%), and do not express CD117, DOG-1, S-100. Conclusion Inflammatory fibrous polyps are rare tumors of the digestive tract. They have characteristic histological changes and immunohistochemistry. They have complete resection under surgery or endoscopy and have a good prognosis.
