基于中医传承辅助平台的针灸治疗中风后肩手综合征选穴组方规律分析

摘　　要：目的运用中医传承辅助平台(V2.5),基于临床文献挖掘,总结针灸治疗中风后肩手综合征的穴位配伍规律,并发掘新处方。方法检索2019年3月31日前中国知网期刊数据库收录的针灸治疗中风后肩手综合征的临床研究文献,将穴位处方录入中医传承辅助平台,运用软件集成的数据挖掘方法,分析其中的穴位配伍规律。结果共纳入143篇文献、173首穴位处方,穴位98个;总体使用频次较高的穴位频次由高到低依次为:曲池、肩髃、合谷、肩贞、外关等;最常见且关联度较高的穴组有"合谷,曲池""肩髃,曲池""肩髃,合谷"等。设置相关度为8、惩罚度为2,得到26组核心穴组和新处方13首。结论针灸治疗中风后肩手综合征的穴组具有重视局部取穴、重用阳经穴、以阳明经穴为重中之重的规律,该选穴规律及新处方可供临床医师参考,为临床治疗该病提供参考方案及新思路。Objective:To summarize the law of acupoint compatibility on shoulder-hand syndrome after apoplexy by acupuncture and moxibustion,and to explore new prescriptions with TCMISS(V2.5),based on clinical literature mining.Methods:The clinical research literature on acupuncture treatment of post-stroke shoulder-hand syndrome included in the CNKI Journal database before 31 March 2019 was searched.The acupoint prescription was put into the TCMISS,and the software integration data mining method was used to analyze the acupoint compatibility.Results:143 articles,173 acupoint prescriptions,and 98 acupuncture points were included.The acupoints with higher frequency of overall use in order of high frequency to low were:Quchi,Jianyu,Hegu,Jianzhen,Waiguan,etc.The most common and highly correlated group of points were Hegu and Quchi,Jianyu and Quchi,Jianyu and Hegu,and so on.The correlation degree was set to 8,the penalty degree to 2;26 sets of core points and13 new prescriptions were got.Conclusion:Acupuncture treatment of shoulder-hand syndrome after stroke has the principle of paying attention to local acupoints,reusing Yang meridian points,and Yangming meridian points as the most important.The selection of acupoints and new prescriptions can be used by clinicians,providing reference programs and new ideas for clinical treatment of the disease.
