缺血性脑卒中后癫痫发生的预测工具评估效能研究

摘　　要：目的本研究用SeLECT量表来评估缺血性卒中后癫痫的发生情况,并对量表进行评价。方法采用回顾性调査的方法,研究2013年1月1日一2014年12月31日益阳市中心医院的缺血性脑卒中患者256例,所有参与者在发病24个月后进行随访,确定在这个时间段内是否发生癫痫,并运用受试者工作特性曲线(receiver operating characteristic curve,ROC)分析SeLECT量表的准确度及最佳临界值。结果本研究共纳入研究对象256例,其中男性143例,女性113例,使用SeLECT评分预测脑梗死2年内癫痫发作,ROC曲线的曲线下面积为0.871,约登指数最大时截点的特异度为0.87,敏感度为0.7。结论SeLECT评分量表是预测中国人群缺血性卒中后癫痫的有效工具。Objective This study used the SeLECT score to assess the incidence of epilepsy after ischemic stroke and to evaluate the scale.Methods A retrospective survey was conducted to study 256 patients with ischemic stroke in the Central Hospital of Yiyang from January 1,2013 to December 31,2014.All participants were followed up after 24 months of the onset to confirm whether the epilepsy occurred during this time period or not.The receiver operating characteristic curve(ROC)curve was used to analyze the accuracy and optimal cutoff of the SeLECT score.RrsuIts A total of 256 subjects were enrolled in the study,including 143 males and 113 females.The SeLECT score was used to predict the ROC curve of seizures within 2 years of cerebral infarction.The area under the ROC curve was 0.871.The specificity is 0.87 and the sensitivity is 0.7.Conclusion The SeLECT socre is an effective tool for predicting epilepsy after ischemic stroke in Chinese population.
