摘 要:目的探讨二乙三胺五乙酸(^99mTc-DTPA)肾动态显像在评价单侧肾积水患者肾功能的临床应用价值。方法收集2017年12月至2018年8月我院治疗的单侧肾积水患者63例,依据肾积水程度将患者分为轻度积水组(18例)、中度积水组(19例)、重度积水组(26例)3组,选择健康成人20例作为对照组,所有研究对象均通过肾动态显像和双血浆法测定肾小球率过滤(GFR),分别记为g-GFR和d-GFR。结果与对照组健侧肾g-GFR相比:不同程度积水患者的患侧肾GRF均减低(P<0.05),且肾功能损伤程度为重度积水组>中度积水组>轻度积水组;在轻度、中度积水患者中g-GFR和d-GFR相关性较高(r=0.802、r=0.697,P<0.001、P=0.001)。结论99mTc-DTPA肾动态显像在不同程度单侧肾积水患者分肾功能评价中具有优势,并且在评估轻中度单侧肾积水患者的总肾功能时更敏感。Objective To investigate the clinical application value of renal dynamic imaging for renal function evaluatio in patients unilateral hydronephrosis.Methods Sixty-three patients with hydronephrosis were enrolled from December 2017 to August 2018,then divided into three groups,which were mild-group(n=18),moderate-group(n=19)and severe-group(n=26),according to degree of unilateral hydronephrosis,and 20 healthy subjects were enrolled at same time as control group.All of the subjects were calculated glomerular filtration rate(GFR)by renal dynamic imaging and two-sample method which were recorded as g-GFR and d-GFR.Results Comparison of g-GFR with healthy kidney in control group:the GRF of the affected kidney was decreased in patients with different degrees of hydronephrosis(P<0.05),among them,severe-group>moderate-group>mild-group.There was a high correlation between g-GFR and d-GFR in patients with mild and moderate hydronephrosis(r=0.802、r=0.697,P<0.001、P=0.001).Conclusion 99mTc-DTPA Renal dynamic imaging has advantages in evaluating unilateral renal function in patients with hydronephrosis and is more sensitive in evaluating total renal function in mild to moderate hydronephrosis.
