摘  要:目的观察补阳还五汤对脑缺血大鼠海马组织Toll样受体4(TLR4)信号及其介导的炎性因子水平的影响,探讨补阳还五汤对脑缺血神经细胞损伤的作用机制。方法将40只雄性Wistar大鼠随机分为正常组、模型组、西药组和中药组,每组10只。模型组、西药组和中药组大鼠均采用双侧颈总动脉阻断法复制脑缺血模型,术后1周,中药组每日灌胃补阳还五汤20 g/kg,西药组每日灌胃尼莫地平10 g/kg,模型组及正常组灌胃给予等容量生理盐水,均1次/d,连续灌胃4周。HE染色观察各组大鼠海马CA1区组织形态学表现,免疫组化法检测海马CA1区TLR4、髓样分化蛋白88(MyD88)表达水平,ELISA法检测海马CA1区肿瘤坏死因子-α(TNF-α)及白细胞介素-1β(IL-1β)水平,Western blot检测海马CA1区核转录因子-κB(NF-κB)表达水平。结果HE染色显示模型组大鼠海马CA1区神经元细胞数量减少,排列疏松不规则,核浓缩,细胞间隙变大;西药组及中药组大鼠海马神经病变明显轻于模型组,细胞数量较模型组多,且形态规则。与正常组比较,模型组大鼠海马组织中TLR4、MyD88蛋白表达水平及TNF-α、IL-1β、NF-κB表达水平均明显升高(P均<0.05);与模型组比较,中药组大鼠海马组织中TLR4、MyD88蛋白表达水平及TNF-α、IL-1β、NF-κB表达水平均明显降低(P均<0.05)。结论补阳还五汤可有效改善脑缺血大鼠的神经损伤,作用机制可能与调节TLR4/MyD88/NF-κB信号、抑制炎症因子表达有关。Objective It is to observe the effect of Buyang Huanwu Decoction on Toll-like receptor 4(TLR4)signaling and its mediated inflammatory factor levels in hippocampus of cerebral ischemia rats,and to explore the mechanism of Buyang Huanwu Decoction on the damage of cerebral ischemic nerve cells.Methods Forty male Wistar rats were randomly divided into normal group,model group,western medicine group and traditional Chinese medicine group,with 10 rats in each group.The rats in the model group,western medicine group and Chinese medicine group were treated with ligation of bilateral common carotid artery to replicate the model of cerebral ischemia.At one week after operation,the Chinese medicine group and western medicine group were respectively intragastrically administered with Buyang Huanwu Decoction 20 g/kg and nimodipine 10 g/kg,the model group and the normal group were given normal volume of normal saline by intragastric administration,all cone per day and treated fore 4 weeks.The histomorphology of hippocampal CA1 was observed by HE staining.The expression of TLR4 and myeloid differentiation protein 88(MyD88)in hippocampal CA1 area was detected by immunohistochemistry.The levels of tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)in hippocampal CA1 area was detected by ELISA.Western blot was used to detect the expression of nuclear factor-κB(NF-κB)in hippocampal CA1 area.Results HE staining showed that the number of neurons in the hippocampal CA1 area of the model group was reduced,the arrangement was loose and irregular,the nucleus was concentrated,and the cell gap became larger.The hippocampal neuropathy of the western medicine group and the Chinese medicine group was significantly lighter and the number of the cells were more with regular cellular histomorphology than that of the model group.Compared with the normal group,the expression levels of TLR4 and MyD88 and the expression levels of TNF-α,IL-1βand NF-κB in the hippocampus of the model group were significantly increased(P<0.05).Compared wit
