摘　　要：目的探讨电子胃镜在儿童疾病中的可行性安全性。方法回顾性分析2016年1月~2019年8月期间南京医科大学第二附属医院接受胃镜检查的199例0~14岁儿童胃镜检查的结果及临床资料。结果 199例儿童共完成231次胃镜操作,共检出病变(包括操作前已确诊的自闭症及消化道异物)195例(98%),全组未发生出血、穿孔、感染及麻醉意外等并发症。腹痛(147/199,73.9%)和呕吐(20/199,10.1%)是胃镜检查最主要的原因,检出病变包括慢性非萎缩性胃炎77例,十二指肠球炎14例,慢性非萎缩性胃炎合并十二指肠球炎33例,十二指肠溃疡22例,胃溃疡7例,嗜酸性细胞胃肠炎3例,胃食管反流8例,上消化道异物8例,自闭症7例,食管狭窄4例,急性糜烂性胃炎6例,过敏性紫癜6例,未见异常4例。结论胃镜在儿童疾病的诊治过程中是安全可行的,具有重要临床使用价值。同时儿科医师应多思考创新,扩展胃镜在儿童非消化系统疾病中的应用。Objective To explore the safety, feasibility, and clinical value of electronic gastroscopy.Methods A retrospective analysis was performed on the data of 199 pediatric patients who were between 0 ~ 14 years old and received gastroscopy from The Second Affiliated Hospital of Nanjing Medical University. Results A total of 199 pediatric patients who had taken 231 gastroscopies and the dectection of diseases in 195 patients(98%).No complications such as bleeding,perforation,infection and anesthesia accident occurred in the whole group.The main indications for gastroscopy included abdominal pain(147/199,73.9%) and vomiting(20/199,10.1%).The detected lesions included 77 cases of chronic non-atrophic gastritis, 14 cases of duodenal inflammation, 33 cases of chronic non-atrophic gastritis and duodenal inflammations, 22 cases of duodenal ulcer, 7 cases of gastric ulcer, 3 cases of eosinophilic gastroenteritis, 8 cases of gastroesophageal reflux, 8 cases of upper digestive tract foreign body, 7 cases of autism, 4 cases of esophageal stricture, 6 cases of acute erosive gastritis, 6 cases of allergic purpura, 4 cases of no abnormalities.Conclusion Gastroscope is safe and feasible in the diagnosis and treatment of childhood diseases, and has important clinical value.At the same time, children’s physicians should think more about innovation and expand the application of gastroscope in children’s non-digestive diseases in order to better serve for pediatric patients.
