摘  要:目的:分析磁共振动脉自旋标记成像技术(3D-ASL)灌注成像标准化肿瘤血流量(nTBF)与胶质瘤病理分级、血管增生程度的相关性,探讨3D-ASL灌注成像在评估胶质瘤患者肿瘤微血管生成情况及协助制订更准确的术前手术方案和术后放疗方案中的运用价值。方法:回顾性分析确诊的34例胶质瘤患者的临床资料。术前行增强MRI及3D-ASL检查,测量胶质瘤病变区肿瘤实质标准化肿瘤血流量,记录肿瘤组织中血管内皮生长因子(VEGF)、ERG蛋白表达程度。分析nTBF值与肿瘤分级、VEGF、EGR表达程度的相关性。结果:中低级别胶质瘤17例(50%),高级别胶质瘤17例(50%),在3D-ASL-CBF图上高级别胶质瘤的灌注明显高于低级别胶质瘤;高级别胶质瘤肿瘤组织nTBF值为(3.16±0.68),低级别胶质瘤肿瘤组织nTBF值为(1.21±0.35),差异有统计学意义(P<0.05);胶质瘤nTBF值与VEGF,ERG的表达均呈正相关。结论:胶质瘤肿瘤实质nTBF与肿瘤分级呈正相关,与VEGF,ERG表达程度密切相关。nTBF值越高,胶质瘤级别越高,VEGF,ERG表达程度也越高。Objective:To investigate the correlation between 3D-ASL perfusion imaging normalized blood flow in tumor(nTBF)values and the pathological scales of glioma and the degree of vascular hyperplasia by the comparation between the values of the normal tumor flow nTBF in glioma parenchyma using 3D-ASL and the expression of vascular endothelial growth factor(VEGF)and ERG protein in tumor tissue,and to explore the profits of the 3D-ASL perfusion imaging in the evaluation of the tumor angiogenesis in glioma patients,so as to assist in making modalities of surgical procedure and post-operation radiotherapy.Methods:Thirty-four patients with glioma were retrospectively analyzed;pre-operative contrast-enhanced MRI and 3D-ASL examinations were used to measure the normalized tumor blood flow nTBF values in the glioma lesions.Postoperative pathological specimens were performed.The immunohistochemical staining was used to evaluate the expression of VEGF,ERG under high-power field of view,protein expression staining scores were grouped according to the glioma WHO levels and analyzed for nTBF values and tumor grades.Correlations between nTBF values and expression levels of VEGF and EGR in tissues were analyzed.Results:Seventeen cases(50%)of low-grade gliomas and 17 cases(50%)of high-grade gliomas were characterized by significantly igher perfusion imaging in high-grade gliomas was significantly higher than that in low-grade gliomas on the 3D-ASL-CBF map.nTBF value of high-grade glioma tumor tissue was(3.16±0.68),nTBF values of low-grade glioma tumor tissue was(1.21±0.35),nTBF in high-grade glioma tumor was higher than that in low-grade group.The expression of nTBF in gliomas was positively correlated with the expression of VEGF and ERG in 34 patients.Conclusion:The glioma tumor nTBF is positively correlated with tumor grade and closely related to the expression of VEGF and ERG.The higher the nTBF value,the higher grade of glioma,and the higher expression of VEGF and ERG.
