摘 要:目的:探讨大剂量沐舒坦联合NCPAP治疗小儿重症肺炎的效果。方法:选取2018年1-12月本院儿科收治的小儿重症肺炎患儿123例作为研究对象,按治疗方法不同分为A组(n=42)、B组(n=41)、C组(n=40),在综合治疗的基础上,A组予以大剂量沐舒坦联合NCPAP治疗,B组单纯予以大剂量沐舒坦,C组单纯予以NCPAP治疗,比较三组的临床疗效。结果:治疗前,A、B、C三组的PaO2、FiO2和OI值比较,差异均无统计学意义(P>0.05);治疗后,A组的PaO2、OI值均高于B、C组,FiO2低于B、C组,差异均有统计学意义(P<0.05)。A组的发绀持续时间、呼吸困难持续时间、住院时间均短于B、C组,差异均有统计学意义(P<0.05)。A组的总有效率显著高于B、C组,差异均有统计学意义(P<0.05)。结论:在小儿重症肺炎的临床治疗中采用大剂量沐舒坦联合NCPAP治疗效果良好,值得临床推广应用。Objective: To investigate the efficacy of high-dose Mucosolvan combined with NCPAP in the treatment of pediatric severe pneumonia. Method: A total of 123 children with severe pneumonia admitted to the department of pediatrics of our hospital from January to December 2018 were selected as the study objects, according to different treatment methods, they were divided into group A(n=42), group B(n=41) and group C(n=40). On the basis of comprehensive treatment, group A received high-dose Mucosolvan combined with NCPAP, group B received high-dose Mucosolvan alone, and group C received NCPAP alone, the clinical efficacy of the three groups was compared. Result: Before treatment, there were no significant difference in the values of PaO2, FiO2 and OI among group A, B and C(P>0.05). After treatment, the PaO2, OI in group A were higher than those in group B and C, and FiO2 was lower than those in group B and C, the differences were statistically significant(P<0.05). The duration of cyanosis, dyspnea and hospitalization in group A were shorter than those in group B and C, and the differences were statistically significant(P<0.05). The total effective rate in the group A was significantly higher than those in the group B and C, the differences were statistically significant(P<0.05). Conclusion: In the clinical treatment of severe pneumonia in children with high-dose of Mucosolvan combined with NCPAP treatment effect is good, worthy of clinical application.
