摘 要:目的:观察更昔洛韦联合异甘草酸镁治疗婴儿巨细胞病毒(CMV)性肝炎的临床效果。方法:选取2012年1月-2018年12月在本院进行治疗的婴儿巨细胞病毒性肝炎患儿60例,使用随机数字表法分为对照组与观察组,各30例。对照组给予包括更昔洛韦在内的常规治疗,观察组在对照组基础上增加异甘草酸镁治疗,比较两组治疗前后肝功能情况、CMV病毒情况以及临床疗效。结果:治疗前两组谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)、直接胆红素(DBIL)以及总胆汁酸(TBA)比较,差异均无统计学意义(P>0.05);治疗后,观察组ALT、AST、TBIL、DBIL、TBA均明显优于对照组(P<0.05)。两组治疗前CMV DNA比较,差异无统计学意义(P>0.05);治疗后观察组CMV DNA明显低于对照组(P<0.05)。观察组治疗后转阴率为76.67%明显高于对照组53.33%(P<0.05)。观察组治疗总有效率为93.33%明显高于对照组70.00%(P<0.05)。结论:更昔洛韦联合异甘草酸镁对婴儿巨细胞病毒性肝炎具有良好的临床疗效,能够改善患儿肝脏功能,提高病毒转阴率,增加治疗的总有效率。Objective: To observe the clinical effect of Ganciclovir combined with Magnesium Isoglycyrrhizinate in the treatment of infant cytomegalovirus hepatitis. Method: A total of 60 cases with infants cytomegalovirus hepatitis received treatment in our hospital from January 2012 to December 2018 were selected and divided into control group and observation group by random number table method, 30 cases in each group. The control group was treated with conventional treatment including Ganciclovir, and the observation group was treated with Magnesium Isoglycyrrhizinate on the basis of the control group. The liver function, CMV virus and clinical efficacy of two group were compared before and after treatment. Result: Before treatment, alanine transaminase(ALT), Rice grass transaminase(AST), total bilirubin(TBIL), direct bilirubin(DBIL) and total bile acid(TBA) of two groups were compared,the differences were not statistically significant(P>0.05);after treatment, ALT, AST, TBIL, DBIL and TBA in the observation group were significantly better than those in the control group(P<0.05). There was no significant difference in CMV DNA between the two groups before treatment(P>0.05);after treatment, CMV DNA in the observation group was significantly lower than that in the control group(P<0.05). The rate of negative conversion in the observation group was 76.67%, which was significantly higher than 53.33% in the control group(P<0.05). The total effective rate in the observation group was 93.33% significantly higher than 70.00% in the control group(P<0.05). Conclusion: Ganciclovir combined with Magnesium Isoglycyrrhizinate has a good clinical effect on infant cytomegalovirus hepatitis, it can improve the liver function of children, improve the negative conversion rate of virus and the total effective rate.
