洛赛克和甲氰咪胍治疗溃疡性出血的疗效对比分析与临床研究

摘　　要：目的研究分析洛赛克和甲氰咪胍治疗溃疡性出血的疗效与临床意义。方法将100例消化性溃疡出血患者,设治疗组和对照组,治疗组:洛赛克80 mg,首次静脉推注。治疗组:洛赛克80 mg,首次静脉推注,继以每小时8 mg静滴维持;凝血酶1000单位溶于37℃温水中,约10 mL,口服或胃管注入。温水不应高于37℃,否则可使溶酶失活,连用5 d。对照组:甲氰咪胍1.2 g加入5%葡萄糖液中静滴;止血敏针3 g,6-氨基已酸针6 g加10%葡萄糖液静滴,每日1次,连用5 d。结果出血停止为有效,若继续出血或血压下降,红细胞计数和血红蛋白下降,脉率增快,则立即手术处理,视为无效。治疗组:出血停止98例,占98%,无效2例,占2%;对照组:有效74例,占74%,无效26例,占26%;两组相比有显著性差异(P<0.05)。治疗组2.6 d,对照组则需4.7 d。结论洛赛克治疗消化性溃疡出血疗效明显优于甲氰咪胍,可减少再出血和急症手术率和提高生存率,值得在临床上推广应用。Objective To study the efficacy and clinical significance of Losec and cimetidine in the treatment of ulcerative hemorrhage.Methods 100 patients with peptic ulcer bleeding,treatment group and control group,treatment group:Losec 80 mg,the first intravenous bolus.Treatment group:Losec 80mg,the first intravenous bolus injection,followed by 8mg per hour intravenous infusion;thrombin 1000 units dissolved in 37℃ warm water,about 10ml,oral or gastric tube injection.Warm water should not be higher than 37℃,otherwise the lysozyme can be inactivated for 5 days.Control group:cimetidine 1.2 g was added to 5% glucose solution for intravenous infusion;hemostatic needle 3 g,6-amino acid needle 6 g plus 10% glucose solution,once daily,for 5 days.Results Bleeding stopped effectively.If bleeding or blood pressure decreased,red blood cell count and hemoglobin decreased,and pulse rate increased,surgical treatment was immediately considered invalid.Treatment group:98 cases of bleeding stopped,accounting for 98%,2 cases were ineffective,accounting for 2%;control group:74 cases were effective,accounting for 74%,and 26 cases were ineffective,accounting for 26%;there was significant difference between the two groups(P<0.05).The treatment group was 2.6 days,and the control group was 4.7 days.Conclusion Losec has a better curative effect on peptic ulcer bleeding than cimetidine,which can reduce the rate of rebleeding and emergency surgery and improve the survival rate.It is worthy of clinical application.
