乳管镜定位下乳腺导管内乳头状瘤切除术的临床应用探讨

摘　　要：目的评价乳管镜定位下乳腺导管内乳头状瘤切除术的临床应用效果。方法选择2014年6月至2018年5月北京大学深圳医院收治的乳腺导管内乳头状瘤患者120例,按随机数字表分为对照组和观察组各60例,对照组予常规乳头状瘤病变区段切除术,观察组予以乳管镜定位下乳腺导管内乳头状瘤切除术,评价两种手术方法效果。结果两组手术时间、切口长度、标本直径、标本重量差异有显著性(P<0.05);观察组视觉模拟疼痛评分低于对照组,差异有显著性(P<0.05);观察组术后病理确诊阳性率(75.00%)高于对照组(56.67%),差异有显著性(P<0.05);但两组治疗费用差异无显著性(P>0.05)。观察组乳房美观度评价优良率高于对照组,差异有显著性(P<0.05);两组并发症发生率、复发率差异无显著性(P>0.05)。结论乳管镜定位下乳腺导管内乳头状瘤切除术治疗,可在精准定位病变同时,提高肿瘤切除术的准确度,减少创伤,应用效果显著。Objective To evaluated breast ductoscopy on the guidance of breast intraductal papilloma surgery.Method 120 patients diagnosed as breast intraductal papiloma in Peking University Shenzhen Hospital from June 2014 to May 2018 were recruited in this study.Patients were randomly divided into two groups:control group and evaluation group based on the table of random number.In control group,traditional selected segmental mastectomy was conducted;while in evaluation group,breast ductoscopy was conducted before surgery for lesion positioning.Result Operation time,incision length,specimen size and weight had a significant difference between two groups(P<0.05).Visual analog pain score on evaluation group is significantly lower than control group(P<0.05).Pathological confirmation of the lesion is much higher in evaluation group(75.00%) vs control group(56.67%).Medical expenses were comparative in both groups.Cosmetic outcome is better in evaluation group(P<0.05).Post operative complication and recurrent rate didn’t achieve significance between two groups(P>0.05).Conclusion Breast ductoscopy guided selected segmental mastectomy for breast intraductal papilloma has the priority of accurate positioning of lesion,which improves surgical accuracy and reduces unnecessary trauma.
