摘  要:目的探讨类风湿关节炎(rheumatoid arthritis,RA)患者外周血单核细胞FcγRⅡb表达及其与骨关节损害的相关性。方法选取2018年1月至12月本院收治的35例初治RA患者纳入观察组,并以1∶1的比例配对选取同期于本院体检的35例健康成人纳入对照组。采用实时荧光定量聚合酶链反应检测两组研究对象外周血单核细胞FcγRⅡb mRNA的表达水平,采用酶联免疫吸附测定检测两组研究对象血清可溶性FcγRⅡb(sFcγRⅡb)、与IgG结合的可溶性FcγRⅡb(sFcγRⅡb-IgG)和抗FcγRIIb抗体含量,分析其与RA患者改良Sharp评分的相关性。结果观察组患者外周血单核细胞FcγRⅡb mRNA表达水平、血清sFcγRⅡb、抗FcγRⅡb抗体水平均显著低于对照组(均P<0.05),血清sFcγRⅡb-IgG水平显著高于对照组(P<0.05)。RA患者外周血单核细胞FcγRⅡb mRNA表达水平、血清sFcγRⅡb、抗FcγRⅡb抗体水平与改良Sharp评分均呈显著负相关(均P<0.05)。结论RA患者外周血单核细胞FcγRⅡb mRNA表达水平降低与骨关节损害显著相关,或可作为判断RA病情严重程度的生物标志物。Objective To investigate the expression of FcγRⅡb in peripheral blood monocytes and its association with joint bone damage in patients with rheumatoid arthritis(RA).Method A total of 35 untreated RA patients admitted to our hospital from January 2018 to December 2018 were recruited in observation group.Thirty-five healthy adults were included in control group in 1∶1 ratio to RA patients.Quantitative real-time polymerase chain reaction(qRT-PCR)was used to detect the expression of FcγRⅡb mRNA in peripheral blood monocytes.Serum levels of total soluble FcγRⅡb(sFcγRⅡb),soluble FcγRⅡb bound to IgG complex(sFcγRⅡb-IgG)and anti-FcγRⅡb autoantibody were detected by enzyme-linked immunosorbent assay(ELISA).Associations between FcγRⅡb mRNA in peripheral blood monocytes,serum levels of sFcγRⅡb,sFcγRⅡb-IgG and anti-FcγRⅡb autoantibody with simplified Sharp score were analyzed in RA patients.Result Expression of FcγRⅡb mRNA in peripheral blood monocytes,serum levels of sFcγRⅡb and anti-FcγRⅡb autoantibody were significantly decreased in RA patients compared with control group(all P<0.05).However,serum sFcγRⅡb-IgG was significantly increased in RA patients compared with control group(P<0.05).The expression level of FcγRⅡb mRNA in peripheral blood monocytes,serum levels of sFcγRⅡb and anti-FcγRⅡb autoantibody were significantly negatively correlated with the modified Sharp score in RA patients(all P<0.05).Conclusion The decreased expression of FcγRⅡb mRNA in peripheral blood monocytes are significantly related to the degree of joint bone damage in RA patients.FcγRⅡb mRNA in peripheral blood monocytes may be a biological marker for predicting disease severity in patients with RA.
