摘 要:目的探讨艾滋病患者CD4^+T细胞水平与机会性感染和病毒载量的相关性。方法选取本院2016年1月至2017年1月收治的128例艾滋病患者为研究对象,测定其血清中的CD4^+T细胞水平和病毒载量,随访6个月,统计发生机会性感染的患者例数和类型,分析CD4^+T细胞与机会性感染和病毒载量的相关性。结果68例患者于6个月内发生机会性感染,感染率为53.12%。最常见的感染类型为细菌性肺炎(30.88%)、肺孢子菌肺炎(22.06%)和口腔念珠菌感染(16.18%)。发生机会性感染或病毒载量≥10^4/ml的患者CD4^+T细胞水平均显著低于未发生机会性感染或病毒载量<104/ml的患者(均P<0.001)。多因素Logistic回归分析显示CD4^+T细胞<100个/μl和病毒载量≥10^4/ml均是艾滋病患者发生机会性感染的危险因素(均P<0.05)。结论CD4^+T细胞水平降低可增加艾滋病患者发生机会性感染的危险,应对其进行常规检测,避免机会性感染的发生。Objective To investigate the relationship between CD4^+ T cell level and opportunistic infection and viral load in acquired immunodeficiency syndrome(AIDS)patients.Method A total of 128 AIDS patients admitted to our hospital from January 2016 to January 2017 were selected for the study.The level of CD4^+ T cells and viral load in serum were measured.Followed up for 6 months,the number and type of patients with opportunistic infection were counted.The correlation between CD4^+ T cells and opportunistic infection and viral load was analyzed.Result 68 patients developed opportunistic infection within 6 months,the infection rate was 53.12%.The most common types of infection were bacterial pneumonia(30.88%),Pneumocystis pneumonia(22.06%)and oral Candida infection(16.18%).The level of CD4^+ T cells in patients with opportunistic infection or viral load≥10^4/ml was significantly lower than those in patients without opportunistic infection or with viral load<10^4/ml(all P<0.001).Multivariate Logistic regression analysis showed that CD4^+ T cells<100/μl and viral load≥10^4/ml were risk factors of opportunistic infection in AIDS patients(all P<0.05).Conclusion The decrease of CD4^+ T cell level can increase the risk of opportunistic infection in AIDS patients.Routine detection should be carried out to avoid the occurrence of opportunistic infection.
