摘 要:目的观察口服高蛋白营养补充剂对有营养风险的老年结直肠癌患者围手术期的营养状况、术后并发症的影响。方法选择河南省某三甲医院拟行结直肠癌手术的60例老年患者,随机分为观察组和对照组,各30例。2组患者术前均进普食,手术当天禁食,术后第3天进流质饮食,第4天进半流质饮食,第5天进普食。观察组在术前3天至术后第7天分别额外给予高蛋白营养补充剂50 g/d。患者均于术前和术后第7天测定血清营养相关指标,并记录术后并发症发生情况。结果(1)观察组使用蛋白粉耐受性较好,但2组不适反应发生率差异无统计学意义(P>0.05)。(2)干预前,2组营养相关生化指标差异无统计学意义(P>0.05);干预后,观察组的前白蛋白、总蛋白、红细胞和血红蛋白均明显高于对照组,差异有统计学意义(P<0.05)。2组术后并发症发生率差异无明显统计学意义(P>0.05)。结论强化蛋白补充能有效改善围手术期老年结直肠癌患者的营养状况,有利于推动胃肠外科加速康复的开展。Objective To observe the effect of oral high protein nutritional supplement on perioperative nutritional status and postoperative complications of elderly patients with colorectal cancer at nutritional risk.Methods A total of 60 elderly patients with colorectal cancer who were scheduled to undergo colorectal cancer surgery in a three-class A hospital in Henan Province were randomly divided into observation group and control group with 30 patients in each group.All the patients in the two groups were given general diet before operation,fasting on the day of operation,liquid diet on the 3rd day after operation,semi-liquid diet on the 4th day after operation and general diet on the 5th day after operation.The observation group was given 50 g/d of high protein nutritional supplement from 3 days before operation to 7 days after operation(except during fasting).Serum nutrition related indicators were measured before and 7 days after operation,and complications were recorded.Results There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).There was no significant difference in nutrition-related biochemical indicators between the two groups before intervention(P>0.05).After intervention,the levels of prealbumin,total protein,erythrocyte and hemoglobin in the observation group were significantly higher than those of the control group(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion Enhanced protein supplementation can effectively improve the nutritional status of elderly patients with colorectal cancer during perioperative period,and promote the accelerated rehabilitation of gastrointestinal surgery.
