摘 要:目的:分析唾液中miR-193a的表达水平与慢性牙周炎的相关性及临床意义。方法:采用qRT-PCR检测慢性牙周炎组(n=100)和健康对照组(n=50)唾液中miR-193a的表达水平;分析健康对照组及轻(n=41)、中(n=37)、重度(n=22)慢性牙周炎组唾液中miR-193a表达水平的相对差异;miR-193a与菌斑指数(PLI)、牙龈指数(GI)、牙周探诊深度(PD)、附着丧失(AL)的相关性。ELISA检测慢性牙周炎组和健康对照组唾液中TNF-α、 IL-6、 IL-8、 IL-14的表达,分析慢性牙周炎组唾液中miR-193a的表达水平与TNF-α、 IL-6、 IL-8、 IL-14的相关性;ROC曲线分析唾液中miR-193a对慢性牙周炎的诊断效能。结果:慢性牙周炎组唾液中miR-193a的表达水平显著低于健康对照组(P<0.05);随着牙周炎症程度的加重,miR-193a的表达水平呈降低趋势,miR-193a与各项牙周临床指标呈负相关;慢性牙周炎组唾液中TNF-α、 IL-6、 IL-8、 IL-14表达水平均显著低于健康对照组(P<0.05);miR-193a与TNF-α、 IL-6、 IL-8、 IL-14呈负相关;ROC显示miR-193a的表达水平具备有效的慢性牙周炎诊断效能。结论:唾液中miR-193a的表达水平降低与慢性牙周炎的进展密切相关,可能成为慢性牙周炎的诊断指标。Objective: To study the correlation and clinical significance of saliva miR-193 a expression with chronic periodontitis. Methods: The expression of miR-193 a in saliva of chronic periodontitis patients(n=100) and healthy controls(n=50) was detected by qRT-PCR. The levels of miR-193 a in saliva of control group, mild(n=41), moderate(n=37) and severe(n=22) periodontitis group were compared. The correlation between miR-193 a and plaque index(PLI), gingival index(GI), probing depth(PD) and attachment loss(AL) was analyzed. The expression of TNF-α, IL-6, IL-8 and IL-14 in saliva of the subjects was measured by ELISA;the correlation between the miR-193 a expression and the cytokins in saliva of the patients was analyzed. The efficacy of miR-193 a in the diagnosis of chronic periodontitis in saliva was analyzed by ROC. Results: The expression of miR-193 a in saliva of chronic periodontitis group was lower than that in control group(P<0.05). The expression level of miR-193 a showed a decreasing trend with the increase of periodontal damage, and the negative correlation between miR-193 a and periodontal clinical parameters. The expression of TNF-α, IL-6, IL-8 and IL-14 in the saliva of periodontitis group was lower than that of the control group;the levels of miR-193 a and the cytokins were negatively correlated;miR-193 a expression levels have effective diagnostic efficacy for chronic periodontitis. Conclusion: miR-193 a level in saliva is negatively related to the progression of chronic periodontitis and can be an diagnostic indicator.
