摘　　要：目的:人乳头状瘤病毒HPVE6/E7mRNA定量检测在宫颈上皮内瘤变诊断中的应用价值。方法:在广西河池市人民医院妇科门诊就诊的宫颈病变患者545例,经TCT检测,按宫颈细胞学TBS系统分级分组为:无上皮内瘤变或恶性病变(NILM)483例,意义不明的不典型鳞状细胞(ASC-US)18例,不能排除高度上皮内病变的不典型鳞状细胞(ASC-H)9例,鳞状上皮内低度病变(LSIL)20例、鳞状上皮内低度病变(HSIL)11例,鳞状细胞癌(ASS)4例。再予以HPVE6E7mRNA拷贝数定量检测,对比分析检测结果,探究临床诊断应用价值。结果:TCT检测结果与HPVE6/E7mRNA定量检测比较表明,TCT检测不同组别随细胞学诊断级别的升高,HPVE6/E7mRNA阳性率逐渐升高(P <0.05)。结论:宫颈上皮内瘤变与HPVE6E7mRNA密切相关,阳性率及拷贝数能够反应病情进展情况,为临床诊断及治疗提供科学指导。Objective: To analyze the value of human papillomavirus E6/E7 mRNA in quantitative detection of cervical intraepithelial neoplasia. Methods: A total of 545 cases of cervical lesions in the department of gynaecology of Guangxi Hechi People’s Hospital were selected and given the TCT detection. According to the cervical cytology TBS system classification they were grouped into: no intraepithelial neoplasia or malignant lesions(NILM) in 483 cases, significance of atypical squamous cells(ASC-US) in 18 cases, 9 cases cannot be ruled out highly intraepithelial lesions in the atypical squamous cells(ASC-H), 20 cases of squamous intraepithelial low-grade lesions(LSIL), squamous intraepithelial low-grade lesions(HSIL) in 11 cases, 4 cases were squamous cell carcinoma(along). Then the HPVE6 E7 mRNA copy number was quantitatively detected, and the test results were compared and analyzed to explore the value of clinical diagnosis. Results: The comparison between TCT results and quantitative detection of HPVE6/E7 mRNA showed that the positive rate of HPVE6/E7 mRNA gradually increased with the increase of cytological diagnostic level in different groups detected by TCT(P < 0.05). Conclusion: Cervical intraepithelial neoplasia is closely related to HPVE6 E7 mRNA, and the positive rate and copy number can reflect the progress of the disease, which provides scientific guidance for clinical diagnosis and treatment.
