颌颈部复合伤15例诊疗分析

摘　　要：目的:探讨颌颈部复合伤的临床特征及救治特点。方法:对15例颌颈部复合伤合并下颌骨粉碎性骨折住院患者进行回顾性分析。在维持生命体征稳定的基础上,通过早期彻底清创,及时行下颌骨骨折切开复位内固定,并加以辅助治疗,术后观察颌颈部外观及咬合功能恢复情况。结果:经6~12个月随访,13例患者咬合功能恢复良好;1例患者咬合功能恢复欠佳,通过调 ,咬合功能得到改善;1例患者颌颈部感染,通过多次换药,瘢痕面积较大,建议患者通过后期整形改善外观。结论:颌颈部复合伤通过及时有效的救治,能最大限度地恢复咬合功能及面颈部美观。Objective:To investigate the clinical characteristics and treatment of compound injury of jaw and neck.Methods:15 inpatients of comminuted mandibular fracture combined with complex trauma of jaw and neck were retrospective analysised.These patients were treated with early thorough debridement,open reduction,and internal fixation of mandibular fracture,after maintaining the stability of vital signs.Adjuvant therapy was adopted,and the appearance and the recovery of occlusal function were evaluated.Results:Followed up for 6-12 months thirteen patients recovered well with normal occlusion function.One case had poor recovery of occlusal function initially,and was improved after adjustment;one patient had local infection,and was left with a large scar area after multiple dressing change and need an elective plastic surgery later.Conclusion:With timely and effective treatment of compound injury of jaw and neck,the occlusal function and esthetics can be restored with satisfactory outcome.
