大承气汤加减保留灌肠治疗不全性肠梗阻临床疗效分析

摘　　要：目的:观察大承气汤加减保留灌肠治疗不全性肠梗阻的临床治疗疗效。方法:研究对象取自本院2016年1月-2018年1月收治的80例不全性肠梗阻患者,将所选患者按照治疗措施的不同,分为观察组40例,使用大承气汤加减保留灌肠治疗,另外40例患者作为对照组使用西药治疗,观察两种治疗措施的临床治疗疗效。结果:观察组患者治疗有效率为95%,对照组患者治疗有效率为75%,观察组患者治疗效果更优;治疗后,观察组患者的排气排便、腹痛、腹胀、恶心呕吐、口干口苦各积分及中医证候总积分均明显低于对照组患者;观察组患者不适症状消失时间、肠鸣音恢复时间、饮食恢复时间以及治疗后住院时间均短于对照组患者;治疗后,在血清C反应蛋白(C-reactive Protein,CRP)、白细胞介素(Interleukin,IL)-6、肿瘤坏死因子-α(Tumor Necrosis Factor,TNF-α)水平方面,观察组患者水平明显低于对照组患者,P <0.05,差异具有统计学意义。结论:对于不全性肠梗阻患者使用大承气汤加减保留灌肠治疗时,在提升临床治疗效果的基础上,对改善临床症状,促进胃肠功能恢复具有显著效果,可以在临床上进行推广使用。Objective: To observe clinical efficacy of enema with the Dachengqi decoction( 大承气汤) on incomplete intestinal obstruction. Methods: 80 cases were divided into two groups, 40 cases in each. The control group was given western medicine;and the observation group was given enema with the Dachengqi decoction. Results: The total efficiency was 95% in the observation group, higher than 75% in the control group. After treatment, the clinical symptoms scores(Flatus and defecation, abdominal pain, bloating, nausea and vomiting, dry mouth and bitterness) and the total scores of TCM syndromes in the observation group were significantly lower than the control group. The disappearance time of discomfort symptoms, recovery time of bowel sounds and diet and hospitalization time after treatment were shorter in the observation group. After treatment, the levels of serum CRP, IL-6 and TNF-α in the observation group were significantly lower than the control group, P<0.05, with statistically significant differences. Conclusion: Enema with the Dachengqi decoction on incomplete intestinal obstruction shows good effects, can improve clinical symptoms, and is worthy of promotion.
