青少年强直性脊柱炎患者疼痛症状与临床指标及其人格特征的相关性

摘　　要：目的探讨青少年强直性脊柱炎患者疼痛与临床指标及其人格特征的相关性.方法对73例青少年强直性脊柱炎患者采用艾森克人格问卷、视觉模拟标尺评分法及自制一般情况调查表进行问卷调查,并进行描述性分析、Pearson相关分析及多元分层回归分析.结果夜间痛与病程、C反应蛋白、Bath强直性脊柱炎患者疾病活动性指数量表评分、Bath强直性脊柱炎患者功能指数量表评分和神经质的人格特征呈显著正相关(r=0.286、0.251、0.780、0.301、0.366,P<0.05或0.01);总体痛与C反应蛋白、Bath强直性脊柱炎患者疾病活动性指数量表评分、Bath强直性脊柱炎患者功能指数量表评分和神经质的人格特征呈显著正相关(r=0.318、0.807、0.260、0.322,P<0.05或0.01).人口学资料、临床指标及人格特征模型共同解释68.9%的夜间痛,79.6%的总体痛.在控制其他变量后,人格也是影响夜间痛和总体痛的主要因素之一(△R^2=0.161、0.146).结论青少年强直性脊柱炎患者的疼痛程度不仅与疾病炎症性指标密切相关,还与神经质的人格特征有关.Objective To investigate the correlation between symptoms and clinical indicators and personality characteristics in adolescent ankylosing spondylitis.Methods By using the Eysenck Personality Questionnaire(EPQ),Visual Analog Scale(VAS)and self-made general condition questionnaire,73 outpatients with AS were investigated.Results Nocturnal pain and disease duration,C-reactive protein,BASDAI,BASFI and neurotic personality characteristics were significantly positively correlated(r=0.286,0.251,0.780,0.301,0.366,P<0.05 or 0.01).Overall pain and C-reactive protein,BASDAI,BASFI,and neurotic personality were significantly positively correlated(r=0.318,0.807,0.260,0.322,P<0.05 or 0.01).Demographic data,clinical indicators,and personality trait models together accounted for 68.9% of nighttime pain and 79.6% of overall pain.After controlling other variables,personality is also one of the main factors affecting nocturnal pain and overall pain(△R^2=0.161,0.146).Conclusions The degree of pain in adolescent ankylosing spondylitis is not only closely related to the inflammatory index of the disease,but also related to the personality characteristics of neuroticism.
