摘　　要：目的本研究旨在评估细胞质多腺苷酸化元件结合蛋白1(CPEB1)mRNA与结直肠癌患者术后转移相关临床结局之间的关系。方法采用荧光实时定量PCR检测221例结直肠癌标本和30例配对正常结肠上皮CPEB1 mRNA表达,并计算肿瘤与正常上皮细胞的表达比值。通过免疫组织化学检测30例结直肠癌患者的CPEB1蛋白表达,并对CPEB1 mRNA比值与临床病理特征与预后的关系进行了评估。结果CPEB1 mRNA水平升高的结直肠癌标本CPEB1 mRNA比值的中位数为1.57(范围为1.01~3.45),而CPEB1 mRNA水平降低者比值为0.53(范围0.23~1.00)。CPEB1蛋白水平表达组织化学评分与CPEB1 mRNA水平显著相关(P=1.43×10-4)。较低的CPEB1 mRNA比值与淋巴结受累显著相关,而肿瘤组织中CPEB1 mRNA比值降低患者的总中位生存时间显著低于升高患者(P=2×10-5)的。CPEB1 mRNA比值降低与无进展生存时间短显着相关(P=0.001)。多变量分析证实,CPEB1 mRNA的下调是总生存期和无进展生存期短的独立危险因素(P=2.31×10-4;P=0.002)。结论较低的CPEB1 mRNA比值是结直肠癌患者预后差的独立预测因子。Objective The aim of this study was to assess the relationship between Cpeb1 mRNA and the postsurgical occurrence of metastasis resulted clinical outcome with colorectal cancer.Methods We examined mRNA of Cpeb1 in 221 colorectal-cancer specimens and 20 adjacent normal colon epithelium samples by using quantitative RT-PCR for mRNA and calculated the ratios of the expression in the tumors and the adjacent normal colon epithelium.Immunoreactivity of CPEB1 expression was measured by immunohistochemistry in a subgroup of 20 colorectal cancer tumors with high or low Cpeb1 mRNA,respectively.Occurrence of metastasis,overall survival and progression-free survival were assessed retrospectively in relation to Cpeb1 mRNA results.Results Levels of Cpeb1 mRNA was variable among cancer specimens;the median ratio of CPEB1 expression in CRC specimens with increased CPEB1 mRNA levels was 1.57(range from 1.01to 3.45)and in those with decreased CPEB1 mRNA levels was 0.53(range from 1.00 to 0.23).The histochemical score of CPEB1 expression was in lines with Cpeb1 mRNA levels(P=1.43×10-4).Lower Cpeb1 mRNA levels were significantly associated with lymph node involvement.The median overall survival was substantially reduced among patients whose tumor had low levels of Cpeb1 mRNA(P=2×10-5).Shorter progression free survival was also significantly associated with lower Cpeb1 mRNA levels(P=0.001).Mutlivariate analysis confirmed that downregulation of Cpeb1 mRNA was associated with overall survival and PFS independent of clinical covariates(P=2.31×10-4 and P=0.002,respectively).Conclusion We found Cpeb1 mRNA to be differentially expressed in colorectal cancer.Lower Cpeb1 mRNA expression may lead to poor clinical outcome in colorectal cancer patients.
