骨化三醇联合米诺环素治疗糖尿病伴牙周炎的临床疗效

摘　　要：目的探讨骨化三醇联合米诺环素治疗糖尿病伴牙周炎的临床疗效。方法选取武汉科技大学附属天佑医院口腔科2017-2018年收治的糖尿病伴牙周炎患者96例,按照随机数字表法分为对照组和观察组,每组48例。对照组给予米诺环素软膏治疗,观察组在对照组基础上给予骨化三醇胶丸治疗,两组均治疗1个月。比较两组治疗前后牙周炎相关指数〔牙龈指数(GI)、牙菌斑指数(PLI)、牙周袋深度(PD)、龈沟出血指数(SBI)〕、炎性反应指标〔C反应蛋白(hs-CRP)、白介素6(IL-6)及肿瘤坏死因子α(TNF-α)〕、糖代谢指标〔空腹血糖(GLU)、糖化血红蛋白(HbA1c)〕,临床疗效,观察两组治疗期间不良反应发生情况。结果治疗前两组GI、PLI、PD、SBI,hs-CRP、IL-6、TNF-α,GLU、HbA1c比较,差异无统计学意义(P>0.05);治疗后观察组GI、PLI、PD、SBI,hs-CRP、IL-6、TNF-α,GLU、HbA1c减轻对照组(P<0.05)。观察组总有效率高于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论骨化三醇联合米诺环素治疗糖尿病伴牙周炎的临床疗效确切,可有效改善患者牙龈情况,减轻感染,控制血糖,且安全性较高。Objective To investigate the clinical effect of calcitriol combined with minocycline for diabetes mellitus with periodontitis.Methods A total of 96 cases of patients with diabetes mellitus and periodontitis were selected from 2017 to 2018 in the Department of Stomatology,Affiliated Tianyou Hospital of Wuhan University of Science and Technology,which were randomly divided into control group and observation group according to random number table,48 cases in each group.The control group was treated with minocycline ointment,while the observation group was treated with calcitriol capsules based on the control group.Both groups were treated for one month.The GI,PLI,PD,SBI,hs-CRP,IL-6,TNF-α,GLU,HbA1 c were compared between the two groups before and after treatment.The clinical effects of the two groups were compared,and the adverse reactions of the two groups were observed during the treatment.Results There was no significant difference in GI,PLI,PD,SBI,hs-CRP,IL-6,TNF-α,GLU or HbA1 c between the two groups before treatment(P>0.05);after treatment,the observation group of GI,PLI,PD,SBI,hs-CRP,IL-6,TNF-α,GLU and HbA1 c were lower than the control group(P<0.05).The observation group of total effective rate was higher than that the control group(P<0.05).There was no significant difference in the incidence rate of adverse reactions between the two groups(P>0.05).Conclusion Calcitriol combined with minocycline for diabetes mellitus with periodontitis has a definite clinical effect,it can effectively improve the gingival condition of patients,ease infection,control blood sugar,and with high safety.
