摘　　要：【目的】探讨血清游离三碘甲腺原氨酸(free tri-iodothyronine,FT3)、血清游离甲状腺素(free thyroxine,FT4)在促甲状腺激素(thyroid stimulating hormone,TSH)在不同急性胰腺炎(acute pancreatitis,AP)患者血清中的表达差异及临床意义。【方法】本院AP患者107例分为胆源性AP(biliary acute pancreatitis,BAP)55例及高脂血症性AP(hyperlipidemic acute pancreatitis,HLAP)52例,进而分为轻度AP(mild acute pancreatitis,MAP)64例、中度AP(moderate severe acute pancreatitis,MSAP)43例,同时选取本院健康体检者50例为对照组。测定各组对象血清中FT3、FT4、TSH水平。【结果】对照组血液Hb、HCT、PLT、AST、GGT、DBIL、CHOL、LDL-C、TG水平低于HLAP组、BAP组(P<0.05);BAP组Hb、HCT、PLT水平低于HLAP组,AST、GGT、DBIL、CHOL、LDLC、TG水平高于HLAP组(P<0.05);对照组、BAP组与HLAP组血液WBC、ALB、TBIL、GLU、Ca、Cr、HDL-C水平差异无统计学意义(P>0.05);HLAP组、BAP组FT3、FT4、TSH水平均高于对照组(P<0.05);BAP组FT4、TSH水平高于HLAP组(P<0.05),HLAP组与BAP组FT3水平差异无统计学意义(P>0.05);MAP组与MSAP组血清FT3、FT4、TSH水平均高于对照组(P<0.05);MSAP组FT3、TSH水平低于MAP组(P<0.05);MSAP组与MAP组血清FT4水平差异无统计学意义(P>0.05);HLAP组中MSAP组血清FT3、TSH水平低于MAP组(P<0.05);而两组血清FT4水平差异无统计学意义(P>0.05);BAP组中MSAP组血清FT3、TSH水平低于MAP组(P<0.05);而两组血清FT4水平差异无统计学意义(P>0.05);HLAP患者血清TSH水平与TG呈负相关(P<0.05),BAP患者血清TSH水平与TG无相关性(P>0.05);低水平的FT3为AP、HLAP、BAP患者发生MSAP的危险因素(P<0.05);FT3预测AP、HLAP、BAP曲线下面积(area under of curve,AUC)为0.748、0.757、0.761。【结论】FT3、FT4、TSH水平随着AP的严重程度而降低,在HLAP患者中尤为明显。FT3可能是早期评估AP严重程度的有用生物标志物,值得在临床上应用。【Objective】To investigate the difference of serum expression of free tri-iodothyronine(FT3),free thyroxine(FT4)and thyroid stimulating hormone(TSH)in patients with different acute pancreatitis and its clinical significance.【Methods】107 patients with acute pancreatitis(AP)were divided into biliary AP(BAP)in 55 cases and hyperlipidemia AP(HLAP)in 52 cases,which were further divided into 64 cases of mild acute pancreatitis(MAP).43 cases of severe acute pancreatitis(MSAP),and 50 healthy subjects in our hospital were selected as the control group.Then the levels of serum FT3,FT4 and TSH were detected in each group.【Results】The levels of Hb,HCT,PLT,AST,GGT,DBIL,CHOL,LDL-C and TG in the control group were significantly lower than those in the HLAP group and the BAP group(P<0.05).The levels of Hb,HCT and PLT in BAP group were lower than that in the HLAP group,and the levels of AST,GGT,DBIL,CHOL,LDL-C,and TG were higher than those in the HLAP group(P<0.05).The levels of WBC,ALB,TBIL,GLU,Ca,Cr and HDL-C was not statistically significant difference among Control group,BAP group and HLAP group(P>0.05).The levels of FT3,FT4 and TSH in HLAP group and BAP group were higher than those in the control group(P<0.05).The levels of FT4 and TSH in BAP group were higher than those in HLAP group(P<0.05).There was no significant difference in FT3 level between HLAP group and BAP group(P>0.05).The level of FT3,FT4 and TSH in MAP group and MSAP group were higher than those of the control group(P<0.05).The level of FT3 and TSH in the MSAP group was lower than that in the MAP group(P<0.05).There was no significant difference in serum FT4 between MSAP group and map group(P>0.05).In the HLAP group,the levels of serum FT3 and TSH in the MSAP group were lower than those in the MAP group(P<0.05),but there was no significant difference in serum FT4 between the two groups(P>0.05).In the BAP group,the levels of serum FT3 and TSH in the MSAP group were lower than those in the MAP group(P<0.05),but there was no significant difference in se
