健脾活血解毒颗粒治疗缓解期溃疡性结肠炎的临床观察

摘　　要：目的:观察健脾活血解毒颗粒治疗缓解期溃疡性结肠炎的临床疗效及作用机制。方法:将56例缓解期溃疡性结肠炎患者随机分为两组。治疗组26例予口服健脾活血解毒颗粒,对照组30例口服柳氮磺吡啶肠溶片治疗,疗程均为3个月。采用症状积分、结肠镜下黏膜形态评分、黏膜活检病理评分及外周血T淋巴细胞亚群比例对两组治疗前后进行评估,并观察两组患者治疗后肠道恢复和复发情况。结果:治疗组总有效率96.15%高于对照组86.67%(P<0.05);6个月后随访,治疗组复发率(8.00%)明显低于对照组(23.08%)(P<0.05)。通过镜下观察患者结肠黏膜形态学及黏膜活检病理形态特征,两组治疗后的结肠黏膜形态、黏膜活检病理评分均较本组治疗前明显改善(P<0.01),且治疗组评分显著低于对照组(P<0.05)。通过流式细胞术检测外周血T淋巴细胞亚群比例,发现两组CD3^+、CD4^+、CD8^+比例较治疗前有所提高,CD4+/CD8+比例下降(P<0.01)。结论:健脾活血解毒颗粒治疗缓解期溃疡性结肠炎的临床疗效显著,值得临床推广运用。Objective:To observe the clinical efficacy and mechanism of Jianpi Huoxue Jiedu Granules in the treatment of ulcerative colitis in remission stage.Methods:Fifty-six patients with ulcerative colitis in remission stage were randomly divided into two groups.The treatment group was treated with Jianpi Huoxue Jiedu Granules and the control group was treated with Sulfasalazine Enteric-coated Tablets for three months.The symptom score,mucosal morphology score with colonoscopy,mucosal biopsy pathological score and peripheral blood T lymphocyte subsets ratio were used to evaluate the two groups before and after treatment.Intestinal recovery and recurrence were observed in both groups after treatment.Results:The total effective rate of the treatment group was 96.15%higher than that of the control group 86.67%(P<0.05),and the recurrence rate of the treatment group(8.00%)was significantly lower than that of the control group(23.08%)(P<0.05)after 6 months of follow-up.The morphology of colonic mucosa and pathological features of mucosal biopsy were observed under microscope,after treatment,the morphology of colonic mucosa and pathological score of mucosal biopsy in both groups were significantly improved compared with those before treatment(P<0.01),and the score in the treatment group was significantly lower than that in the control group(P<0.05).Flow cytometry was used to detect the percentage of T lymphocyte subsets in peripheral blood of UC patients in remission stage,it was found that the percentage of CD3^+,CD4^+,CD8^+ in treatment group was higher than that before treatment,and the ratio of CD4^+/CD8^+ was lower(P<0.01).Conclusion:Jianpi Huoxue Jiedu Granule is effective in treating ulcerative colitis in remission stage,which is worthy of clinical application.
