摘　　要：目的:了解磁县地区无症状人群上消化道恶性肿瘤及各级别癌前病变的发生情况。方法:在2005~2018年期间,连续13年对磁县7个乡镇27 994例40~69岁的无症状人群进行胃镜普查,并取病变部位活检组织,进行病理学检查和诊断。结果:检出食管癌、贲门癌、胃癌及其各级别癌前病变患者各2 819例、343例、213例,其中食管癌、贲门癌、胃癌患者分别为234例、95例、26例,食管癌和贲门癌发病人数占该地区上消化道恶性肿瘤的92. 7%;对应早期癌分别为194例、56例、20例,占相应恶性肿瘤的83%、59%、77%。结论:食管癌、贲门癌是严重威胁该地区人民群众身体健康的上消化道恶性肿瘤,无症状人群内镜普查极大地提高了上消化道恶性肿瘤的早期诊断率及救治率,为本地区肿瘤防控的重要方法和手段。Objective:To investigate the occurrence of upper gastrointestinal cancer and precancerous lesions in asymptomatic population in Ci County.Methods:From 2005 to 2018,People’s Hospital of Ci County carried out gastroscopy for27,994 asymptomatic people aged 40-69 in 7 villages in Ci County.Lesion biopsy was taken for pathological examination and diagnosis.Results:The number of new cases of esophageal cancer,gastric cardia cancer and gastric cancer were 243,95 and 26,that of those cancers’precancerous lesions were 2,819,345 and 213,and that of early esophageal cancer,gastric cardia cancer and gastric cancer were 194,56 and 20,which accounting for 83%,59%and 77%of their corresponding cancers.Esophageal cancer and gastric cardia cancer accounted for 92.7%of upper gastrointestinal cancers in this area.Conclusion:Esophageal cancer and gastric cardia cancer are upper gastrointestinal cancers that seriously threaten the physical and mental health of people in this region.Endoscopic screening of asymptomatic population,which is an important measure for cancer prevention in the region,has greatly improved the early diagnosis and treatment of upper gastrointestinal cancers.
