单颗牙种植修复的影响因素分析

摘　　要：目的:调查单颗牙拔除后患者种植修复的实行情况及进行相关影响因素分析,为提高种植修复率提供指导。方法:对2017年8—10月于广西医科大学附属口腔医院颌面外科拔除单颗牙并需要修复的202例患者进行口腔检查和现场问卷调查,并于2018年8—10月电话回访其是否实行种植修复;分别采用卡方检验和多因素非条件Logistic回归分析模型进行患者种植修复的单因素和多因素分析。结果:共发放调查问卷202份,1年后成功回访162份,回访率为80.2%。拔牙后1年单颗牙缺失种植修复率为25.3%;单因素分析结果显示,患者年龄、受教育程度、月收入、是否有口腔修复治疗史、拔牙牙位、向患者介绍修复方案医生性别、职称、是否有种植经验、患者对种植认知水平跟种植修复率有关;多因素Logistic回归分析结果显示,对种植的认知水平低、介绍修复方案医生无种植经验和患者月收入低的患者行单颗牙种植修复可能性越小;其中患者获取种植认知的途径主要是通过医生介绍。结论:在有修复必要的单颗牙缺失患者中,种植修复率不高,实行种植修复的影响因素主要是种植认知水平、医生是否有种植经验和月收入。Objective:To investigate the implant restoration rate and analyze the related factors after single tooth extraction,and to provide guidance for improving the implant restoration rate.Methods:From August to October 2017,202 patients who had undergone single tooth extraction at the Department of Maxillofacial Surgery of the Affiliated Stomatological Hospital of Guangxi Medical University and needed restoration were oral examined and surveyed,and a telephone follow-up was conducted between August and October 2018 to confirm whether they had received implant restoration.The results were analyzed by Chi-square test and multivariate logistic regression.Results:A total of 202 questionnaires were issued,and 162 patients were followed up after one year.The follow-up rate was 80.2%.The restoration rate after one year of single tooth extraction was 25.3%.Univariate analysis showed that the patient's age,education level,income,history of oral prosthetic treatment,missing tooth position,the dentist's job title,whether the dentist had implant experience,and the patient's cognitive level were related to the implant restoration rate.Multivariate logistic regression analysis showed that patients with low implant cognitive level,patients introduced by dentists with less implant experience,and patients with low monthly income were related to lower single tooth implant rate.The way patients gain implant cognition was mainly through dentists.Conclu sion:The single tooth implant rate was not high,mainly due to low patient income,poor oral conditions,and low level of implant cognition.
