缺氧缺血性脑病患儿血清8-OHDG、VILIP-1和NSE水平变化及其与NBNA评分的相关性分析

摘　　要：目的:探讨新生儿缺氧缺血性脑病(HIE)患儿血清8-羟基脱氧鸟苷(8-OHDG)、视椎蛋白样蛋白1(VILIP-1)和神经元特异性烯醇化酶(NSE)水平变化及与新生儿行为神经(NBNA)评分的相关性。方法:选择2017年3月至2019年3月四川省达州市中西医结合医院收治的HIE患儿117例作为观察组,按不同病情程度分为轻度组(57例)、中度组(39例)和重度组(21例);同期选择本院正常新生儿60例作为对照组。采用化学发光法检测各组血清8-OHDG和NSE水平,酶联免疫吸附试验(ELISA)法测定血清VILIP-1水平。采用Pearson相关分析方法分析血清8-OHDG、VILIP-1和NSE水平与NBNA评分的相关性。结果:观察组血清8-OHDG、VILIP-1、NSE水平显著高于对照组,而NBNA评分显著低于对照组(均P<0.05)。重度组血清8-OHDG、VILIP-1和NSE水平高于中度组和轻度组,且中度组高于轻度组(P<0.05)。重度组NBNA评分低于中度组和轻度组,且中度组低于轻度组(P<0.05)。血清8-OHDG、VILIP-1和NSE水平与NBNA评分均呈负相关关系(r=-0.783、-0.716、-0.854,均P<0.05)。结论:HIE患儿血清8-OHDG、VILIP-1和NSE水平升高,且与NBNA评分呈负相关关系;8-OHDG、VILIP-1和NSE可作为评价HIE患儿病情程度的参考指标。Objective:To investigate the changes of serum 8-hydroxydeoxyguanosine(8-OHDG),vertebral protein-like protein-1(VILIP-1)and neuron-specific enolase(NSE)levels in neonates with hypoxic-ischemic encephalopathy(HIE)and their correlation with neonatal behavioral neurological(NBNA)score.Methods:A total of 117 children with HIE admitted to our hospital from March 2017 to March 2019 were selected as an observation group,and divided into mild group(57 cases),moderate group(39 cases)and severe group(21 cases)according to different degree of the disease.And 60 healthy newborns were selected as a control group.Serum 8-OHDG and NSE levels were detected by chemiluminescence method,and serum VILIP-1 level was determined by enzyme-linked immunosorbent assay(ELISA).The correlation between serum 8-OHDG,VILIP-1 and NSE contents and NBNA score was analyzed.Results:The serum levels of 8-OHDG,VILIP-1 and NSE in the observation group were higher than those in the control group(P<0.05).The NBNA score of the observation group was higher than that of the control group(P<0.05).Serum 8-OHDG,VILIP-1 and NSE levels in severe group were higher than those in moderate and mild groups,and the NBNA score was higher(P<0.05).Serum 8-OHDG,VILIP-1 and NSE levels were negatively correlated with NBNAscore(r=-0.783,-0.716,-0.854,P<0.05).Conclusion:The levels of 8-OHDG,VILIP-1 and NSE in children with HIE were elevated and negatively correlated with NBNA scores;8-OHDG,VILIP-1 and NSE can be used as references for evaluating the severity of HIE.
