实时超声引导下肾穿刺活检的临床应用

摘　　要：目的:探讨超声实时引导下肾穿刺活检的操作技巧,以提高穿刺活检成功率,减少并发症的发生。方法:2016年1月-2018年4月选取行超声实时引导下肾穿刺活检患者105例,对比分析穿刺针数、成功取得肾小球个数及并发症发生情况。结果:105例患者穿刺2针成功95例(90.48%),穿刺3针成功10例(9.52%)。获取肾小球数10~20多个。术后并发腰痛或腰部不适15例(14.29%),肉眼血尿2例(1.90%),肾周血肿6例(5.71%)。结论:准确定位引导在超声实时引导肾穿刺活检中,对提高肾活检成功率、减少并发症具有重要作用。Objective:To discuss the operation skill of renal biopsies guided by ultrasound in real-time,to improve the success rate of aspiration biopsy and reduce the incidence of complications.Methods:From Jan 2016 to Apr 2018,105 cases of renal biopsies guided by ultrasound in real-time were selected,to comparative analysis the numbers of puncture needle,the numbers of glomerulus which were sucessfully getted,the occurrence of complications.Results:Among the 105 cases,95 cases were successfully punctured two needles(90.48%),10 cases were successfully punctured three needles(9.52%).Obtained 10 to 20 glomerulus.15 cases(14.29%)were complicated by lumbago or waist discomfort postoperative,2 cases(1.90%)of gross hematuria,and 6 cases(5.71%)of perirenal hematoma.Conclusion:Accurate location guidance plays an important role in renal biopsies guided by ultrasound in real-time for improving renal biopsy success rate and reducing complications.
