摘　　要：目的探讨ProTaper机械镍钛锉根管预备在牙髓病或根尖周病患者中的应用效果。方法选择2016年4月至2019年5月汉中市中心医院口腔科接诊的174例牙髓病或根尖周病患者为研究对象,共179颗患牙,按随机数表法将患者分为观察组和对照组各87例,对照组患者予以手持不锈钢K锉进行根管预备,观察组患者予以ProTaper机械镍钛锉进行根管预备。比较两组患者的根管预备及填充时间、根管充填质量及预备后疼痛反应。结果观察组患者的根管预备及填充时间分别为(51.35±4.88)min、(23.32±5.63)min,明显短于对照组的(62.51±4.30)min、(52.31±5.09)min,差异均有统计学意义(P<0.05);观察组患者的根管恰填率为91.95%,明显高于对照组的79.31%,差异有统计学意义(P<0.05);观察组患者的疼痛反应率为4.60%,明显低于对照组的25.29%,差异有统计学意义(P<0.05)。结论ProTaper机械镍钛锉根管预备在牙髓病或根尖周病患者中的应用效果满意,其能缩短根管预备及填充时间,有效提高根管充填质量,降低患者疼痛反应率,操作简便且安全高效,值得临床借鉴。Objective To investigate the application of ProTaper mechanical nickel-titanium root canal preparation in patients with endodontic or periapical diseases. Methods A total of 174 patients with endodontic or periapical diseases(179 teeth), who admitted to Department of Stomatology, Hanzhong Central Hospital from April 2016 to May2019 were selected and divided into the observation group and control group according to random number table method,with 87 patients in each group. The patients in the control group were given a stainless steel K file for root canal preparation, and the patients in the observation group were treated with Pro Taper mechanical nickel-titanium file for root canal preparation. The root canal preparation and filling time, root canal filling quality and post-preparation pain response between the two groups were compared. Results The root canal preparation and filling time of the observation group were(51.35±4.88) min,(23.32±5.63) min, respectively, which were significantly shorter than corresponding(62.51±4.30) min and(52.31±5.09) min of the control group(all P<0.05);the rate of root canal filling in the observation group was 91.95%,which was significantly higher than 79.31% in the control group(P<0.05);the pain response rate of the observation group was 4.60%, which was significantly lower than 25.29% of the control group(P<0.05). Conclusion The application of Pro Taper mechanical nickel-titanium root canal preparation is satisfactory in patients with endodontic or periapical diseases. It can shorten root canal preparation and filling time, effectively improve root canal filling quality, reduce pain response rate of patients, and is simple, safe and efficient. It is worthy of clinical application and promotion.
