摘 要:目的探讨多肿瘤标志物蛋白芯片(C12芯片)连续3年筛查对肿瘤检出率的影响。方法收集某高校40周岁及以上身体键康、无肿瘤病史教师2013-2015年连续3年的健康体检C12芯片检测结果,分析C12芯片连续筛查对肿瘤检出率的影响。按照年龄分为中年组(40~59岁)、老年组(60~79岁)和高龄组(≥80岁),比较不同性别和年龄组肿瘤标志物的阳性率和检测值的特点。结果共3634名教师完成了连续3年的C12芯片检测及相应的随访。汇总11017例的C12芯片检测结果分析表明,其中1027例有1项或多项标志物阳性,阳性率为9.32%。中年组、老年组和高龄组体检者肿瘤标志物总阳性率分别为4.45%、10.84%和26.97%,随年龄的增大而升高(P<0.01)。再次检测后仍超过正常值2倍以上的受检者行进一步检查,肿瘤指标单次,连续2次和连续3次阳性的体检者,肿瘤检出率分别为1.46%,6.07%和7.08%,肿瘤检出率随肿瘤标志物持续阳性次数的增多而增大(P<0.01)。结论C12芯片检测的肿瘤标志物总阳性率和肿瘤检出率随年龄增大而增高,肿瘤检出率随肿瘤标志物持续阳性次数的增多而增大,C12芯片连续检测有助于提高肿瘤检出率。Objective To evaluate the effect of tumor detection rate by three?year continuous use of multi?tumor markers protein biochip(C12 chip).Methods Initial cohort contained aged 40 years and over without can?cer history was offered C12 chip assay for cancer screening in continuous three years from 2013 to 2015.Twelve tu?mor markers including alpha fetoprotein(AFP),carcinoembryonic antigen(CEA),carbohydrate antigen 15?3(CA15?3),carbohydrate antigen 19?9(CA19?9),carbohydrate antigen 125(CA125),carbohydrate antigen 242(CA242),ferritin,human chorionic gonadotropin beta(β?HCG),human growth hormone(HGH),neuron spe?cific enolase(NSE),prostate special antigen(PSA)and free PSA(f?PSA)were measured simultaneously by C12 chip.The cohort of all 3 years was consolidated and then classified into three groups according to age.The year range of middle?age group,elderly group was 40~59 years and 60~79 years,while senile group was 80 years and over,respectively.Results 3634 persons accomplished the three?year C12 assay and follow?up.In 3 years,1027 of 11017 cases were positive for one or more markers,and the positive rate was 9.32%.The total positive rate of middle aged group,elderly group and advanced age group was 4.45%,10.84%and 26.97%,respectively,the indicating rate was increased with age(P<0.01).Tumor detection rates increased with times of continuous positive results of tumor markers(P<0.01).The tumor detection rates of persons had once,consecutive 2 and 3 times of positive results were 1.46%,6.07%,and 7.08%,respectively.Conclusions The total positive rate of tumor markers and tumor detection rate evaluated by C12 chip assay was increased with age.Tumor detection rates in?creased with times of continuous positive results of tumor markers.C12 chip consecutive assay can help to increase the detection of tumors rate.
