脑室型颅内压监测在特重型颅脑损伤中的临床应用

摘　　要：目的分析对特重型颅脑损伤患者进行脑室型颅内压监测的作用。方法以2017年1月-2019年5月98例特重型颅脑损伤患者为对象,随机将患者分为两组,其中49例患者通过保守方法监测颅内压,视作对照组,另外49例患者接受脑室型颅内压监测仪监测,视作观察组,比较两组脱水剂使用时间、脱水剂使用剂量、住院时间、预后。结果观察组甘露醇使用剂量、甘露醇持续使用时间、脱水持续时间均优于对照组(P<0.05);观察组ICU住院时间与总住院时间短于对照组(P<0.05);观察组术后GOS 1分、2分、4分、5分比重分别为6.12%、12.24%、40.82%、22.45%,优于对照组20.41%、28.57%、20.41%、4.08%(χ~2=3.874、4.021、4.804、7.184,P<0.05)。结论脑室型颅内压监测用于特重型颅脑损伤中能够减少甘露醇使用量,缩短住院时间,改善患者预后。ObjectiveTo analyze the effect of ventricular intracranial pressure monitoring in patients with severe craniocerebral injury.Methods 98 patients with severe craniocerebral injury from January 2017 to May 2019 were randomly divided into two groups.49 patients were treated as control group by conservative monitoring of intracranial pressure,and the other 49 patients were monitored by ventricular intracranial pressure monitor as observation group.The use time,dosage,hospitalization time and prognosis of dehydrating agent were compared between the two groups.Results The dosage of mannitol in the observation group,the duration of mannitol use and the duration of dehydration were better than those in the control group(P<0.05).The ICU stay time and total hospitalization time in the observation group were shorter than those in the control group(P<0.05).GOS scores of 1 point,2 points,4 points,and 5 points were 6.12%,12.24%,40.82%,and 22.45%,respectively,which were better than the control group 20.41%,28.57%,20.41%,and 4.08%(χ2=3.874、4.021、4.804、7.184,P<0.05﹚.ConclusionThe monitoring of ventricular intracranial pressure can reduce the use of mannitol,shorten the hospitalization time and improve the patients with severe craniocerebral injury.
